{"title":"Gestational Weight Gain and Large for Gestational Age Neonates in a Predominantly Hispanic Population Community Hospital","authors":"Brittany N Robles","doi":"10.33552/wjgwh.2020.03.000572","DOIUrl":"https://doi.org/10.33552/wjgwh.2020.03.000572","url":null,"abstract":"To analyse changes in Gestational Weight Gain (GWG) in different Body Mass Index (BMI) categories at start of pregnancy and neonatal birth weight outcomes in a predominantly Hispanic population living in a low-income environment. We conducted a cross sectional study of women with singleton gestation who delivered at Wyckoff Heights Medical Centre from January 1 st to December 31 st , 2017. BMI was categorized at first prenatal visit as normal weight (BMI= 18.5-24.9), overweight (BMI= 25-29.9) and obese (BMI >30). BMI was collected at 16-20, 24-28, and 36-38 weeks of gestation. Stratified by BMI, appropriate GWG were 25–35 pounds for normal weight (27.9%), 15–25 pounds for overweight (34.5%), and 11–20 pounds for obese (24.5%). Neonatal birth weight was categorized by Duryea percentiles and gestational age in weeks. From 831 women, normal weight (n=269), overweight (n=263), and obese (n=299) women were found. GWG was categorized as: inadequate, appropriate, or excessive based on the Institute of Medicine guidelines. The prevalence of excessive GWG was 23.1% for normal weight, 35.8% for overweight, and 37.5% for obese women. The prevalence of inadequate GWG was 48.9% for normal weight, 31.0% for overweight, and 40.6% for obese women. A significant association was found between obese women and >90 percentile neonatal birth weight (OR:2.3, 95% CI: 1.15-4.97). {"title":"Assessment of the Immunomodulatory Role of Dydrogesterone in Preventing Pregnancy Loss in Threatened Abortion","authors":"O. Materials and methods: This were a prospective interventional study that included two groups each included 16 women. Group 1 included 16 pregnant women presented with symptoms of threatened abortion, and group 2 that included 16 health females with normal pregnancy. All of the studied females were subjected to complete clinical assessment. Laboratory assessment of serum levels of IL-4 (Marker of T-helper 2), IFN γ (Marker of T-helper 1) and serum progesterone level were done for all participants. All women in group 1 were treated with certain doses of Dydrogesterone while women in control group were not allowed to use any form of progesterone. Participants were assessed for IL-4, IFN-γ and progesterone after treatment. Also continuing pregnancy rates were calculated. Results: Both groups were matched as regarding patients’ characteristics and baseline parameters. Dydrogesterone has resulted in significant decrease of Th1/Th2 ratio with shift from Th1 predominance to Th2 predominance. Continuing pregnancy rate in the threatened abortion group was 93.8%. In cases of female patients of reproductive age, the ultimate goal remains the fertility preservation and the quality of life of the patient. According to recent bibliography, bizarre leiomyomas remain a controversial issue regarding the preoperative and postoperative therapeutic mapping. Giant cells with pleomorphic nuclei and little or no mitotic activity compose the microscopic analysis of such lesions. Multidisciplinary approach is mandatory in order to establish ultimate diagnosis and treatment. Laparoscopic salpingectomy and salpingotomy are the common ways to treat tubal pregnancies based on the damage degree of the tubes. Repeated ectopic pregnancies after unilateral or bilateral salpingectomy are rare reported but not uncommon in assisted reproductive technology (ART). This case report presents three women have had unilateral/bilateral laparoscopic salpingectomy due to at least two previous tubal pregnancies, but they experience repeated tubal pregnancies during their ART treatment. We suggest that the gynecologists should take carefully consideration on patient’s future fertility when performing the surgical treatment on tubal pregnancies. Salpingectomy without proximal tube ligation potentially increase the risk of residual tubal pregnancy in ART treatment. Further, the frequency of vasoocclusive pain crises increases during pregnancy, which is thought to be due the associated immune modulation, hypercoagulability, and necessary suspension of hydroxyurea use [3]. Studies have indicated that women with sickle cell disease also experience greater degrees of dysmenorrhea and pains distinct from dysmenorrhea during menses [4,5]. Females with menstruationinduced vaso-occlusive crises often have heavier, longer menstrual periods [4]. Design: Clinical randomized controlled study. Methods: 100 pregnant patients were selected suffering from PROM , and prepared for termination of pregnancy either normal or cesarean , all had no clinical signs of chorioamnoitis , blood markers ( CRP, TL, N/L ratio and P/L ratio) were taken before termination , the results statistically analyzed according to the result of pathological examination of amniotic membrane for detection of early inflammatory signs for chorioamnoitis. Results: There was significant relationship for the P/L ratio and the finding of early chorioamnoitis by pathological examination with P value 0.0004 , however other markers failed to show any significant relationship with early chorioamnoitis , But CRP was found to have a significant relationship with the presence of postpartum complications with P value 0.0003 and other markers had insignificant relationship, There was insignificant relationship with all the four markers with the method of delivery , CRP and TLC had lower specificity in detection of subclinical chorioamnoitis as compared by N/L ratio and P/L ratio , also P/L ratio had the higher sensitivity 85.71% , so by comparing the four markers P/L ratio is the most accurate 90% and N/L ratio had accuracy more than TLC and CRP ( 80%).
