The aim of this article is to describe a method for maximizing tissue response in the treatment of selected subjects with anterior open bites.</p><p><strong>Method: </strong>Twelve subjects with an anterior open bite and insufficient incisor display underwent a limited corticotomy to augment alveolar bone response by creating an anteroposterior intra-arch anchorage differential. Cephalometric radiographs were taken before treatment (T1) and 1 month after open bite closure (T2). The change in perceived smile esthetics was assessed by a visual analog scale.</p><p><strong>Results: </strong>All open bites were closed after a mean of 6 weeks. Radiographic evaluation showed significant maxillary incisor extrusion and retrusion. {"title":"Long-term follow-up of a young adult patient with a Class II malocclusion treated with a Herbst appliance.","authors":"Raffaele Schiavoni, Vittorio Grenga","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A 13-year, 9-month-old girl presented with the chief complaint of crowding and protrusion of her anterior teeth. She was dolichofacial with a Class II Division 1 occlusion and had reached the final phase of her skeletal growth. Clinical symptoms and tomograms were indicative of a TMJ dysfunction. Because orthognathic surgery was not accepted by the patient and her parents, a Herbst appliance became the choice of treatment. An optimal occlusion and a good facial appearance were achieved after 24 months of treatment. TMJ tomograms at the end of treatment showed a more physiologic condylar position. The treatment resulted in a Class I occlusion and ideal overjet, overbite, and incisor angulation. Class I occlusion was achieved after 6 months, and total treatment time was 1 year 6 months. Although the initial clinical signs are pain and discomfort, synovial changes progressively involve all the axial joints, including the temporomandibular joint (TMJ). Eventually, bony alterations develop (condylar erosions, flattening, sclerosis) that affect the position of the condyle, the superior joint space, and the range of movements. These symptoms correlate with the severity of the disease. Besides physiotherapy and surgery, no dental rehabilitation has been reported for these patients. This report of a female patient with ankylosing spondylitis and a TMJ disorder emphasizes dental rehabilitation. The aim of the splint, orthodontic, and prosthodontic treatment was to relieve the subjective symptoms through establishing a stable optimum occlusion. {"title":"Comparison of the frontal esthetic preferences in the lower facial portion of Koreans and Caucasians.","authors":"Yoon-Goo Kang, Yong-Hun Lee, Yoon-Ah Kook, Seong-Hun Kim, Peter M Sinclair","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Aim: </strong>To compare the differences in frontal esthetics in the lower face among Caucasian models, Korean models, and Korean nonmodels.</p><p><strong>Methods: </strong>The sample consisted of three groups comprising 36 Caucasian models, 30 Korean models, and 26 Korean nonmodels. Their frontal photos were scanned, and 12 distances, five angles, lip perimeter, and lip area were measured. Differences among groups were compared using one-way analysis of variance (ANOVA) and Tukey's test.</p><p><strong>Results: </strong>There was no significant difference in the upper lip height among the three groups. The Korean models had shorter lower lip to chin height than the two other groups. Full lip width was wider in the Korean and Caucasian models than in the Korean nonmodels. The Caucasian models had greater total vermilion area compared to both Korean groups. Finally, the Korean models had fuller lower and thinner upper vermilion areas than the nonmodels.</p><p><strong>Conclusions: </strong>Attractive lower facial portions in Caucasians are different from those of Koreans. The therapy included four steps: (1) periodontal flap surgery and root planing with regular follow-ups; (2) leveling and aligning of the maxillary and mandibular arch with closure of all spaces, the extraction of mandibular first premolars, and retraction of the mandibular incisors; (3) LeFort I two-segment osteotomy with removal of the second premolars and a 5-mm impaction to reduce the excessive gingival display and permit autorotation of the mandible to decrease the facial convexity. To close the extraction space, the posterior segment was set 4 mm anteriorly and the anterior segment 3 mm posteriorly; and (4) postsurgical orthodontics for finishing and detailing. Surgically assisted rapid maxillary expansion was followed by maxillary protraction with a face mask and orthodontic treatment with standard edgewise technique.
