In addition, the prevalence of elderly onset epilepsy is estimated to be about 1%. Non - cirrhotic portal - systemic encephalopathy often develops in middle - aged and elderly people. It presents with various symptoms, but rarely with epileptic seizures. We experienced a case of portal - systemic encephalopathy with epileptic seizures in an elderly patient. A 79 - year - old woman presented with epileptic seizures. The symptom improved rap-idly with the administration of antiepileptic drugs, but hyperammonemia was present. Urease - produc-ing Klebsiella oxytoca was detected in urine culture, and trimethoprim - sulfamethoxazole combination was administered, but hyperammonemia persisted. Electroencephalography showed a persistent general-ized slow wave. Head MRI showed a high - intensity area in the bilateral globus pallidus on T1 - weighted images, and abdominal CT showed a portal - systemic shunt that branched from the portal vein and flowed 身長 142 cm ,体重 directly into the inferior vena cava. The patient was diagnosed with portal - systemic encephalopathy and the ammonia level was normalized by conservative treatment. When treating patients with the first epileptic seizure occurring at older ages, it is necessary to distinguish between elderly onset epilepsy and other diseases that cause epileptic seizures. In Japan, a super - aging society, understanding elderly onset epilepsy is important.","PeriodicalId":86662,"journal":{"name":"[Fukushima igaku zasshi] Fukushima medical journal","volume":"95 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"75966235","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We have been studying the mechanism of oocyte aging in animal experiments. There are two types of oocyte aging : pre - ovulatory and post - ovulatory. Both kinds of oocyte aging cause deterioration in embryonic development. We found that, in post - ovulatory oocyte aging, mitochondrial dysfunction due to oxidative stress is the leading cause of the decline in oocyte quality. However, a decrease in mitochondrial function was observed in preovulatory aged oocytes, which was not associated with the increased oxidative stress. It was sug-gested that different molecular mechanisms may be involved in the deterioration of embryonic development in post - ovulatory and pre - ovulatory aged eggs. At present, there is no effective treatment for age - related fertility decline, and the only remedy is oocyte cryopreservation at a young age. Oocyte cryopreservation has been done for two reasons : a medical indication to prevent the loss of ovarian function due to cancer treatment and a non - medical indication for healthy women to prevent future loss of fertility. While there are some advantages in the latter case, it is not recommended due to the risks associated with older pregnancies and complications for the mother and child. In this paper, I review the disease of infertility from both biological and sociological perspectives and outline the current status of infertility and its problems.","PeriodicalId":86662,"journal":{"name":"[Fukushima igaku zasshi] Fukushima medical journal","volume":"32 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"81247719","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
