Though it is rare, it is pertinent for clinicians to know about this condition so as to improve their ability to manage it and to note that management is multidisciplinary. We present a case of Apert Syndrome in a one month old Nigerian female, and one of a set of twins that presented with proptosis, hypertelorism, high arched palate and fusion of bones of fingers and toes. {"title":"Seroprevalence of Hepatitis C Viral Antibodies among Pregnant Women in a Health Facility in Niger-Delta, Nigeria","authors":"B. Utoo, P. Some infected patients are known to develop liver cirrhosis and /or possible hepatocellular carcinoma. This study was designed to find out the seroprevalence of hepatitis C viral antibodies among antenatal attendees in a mission hospital in Niger-Delta, Nigeria. This was a cross-sectional study in which ELISA test was done on consented consecutive antenatal attendees from 1st January to 30th June 2010. Eight hundred and thirty six (836) pregnant women were counselled and screened for HCV antibodies. Out of this number, 24(2.87%) tested positive for HCV antibodies. Their mean age and parity were 25.5 ± 4.5 years and 2.2 ± 1.1 respectively. Most (75%) of the sero-positive subjects had little level or no formal education; were older in age (37.5%) and mostly (37.5%) engaged in commercial activities. Education, public health enlightenment, women empowerment, modification of risky behaviors among other intervention measures could help to reduce the high rate of HCV infection observed in the study. {"title":"Bacterial Isolates from Cerebrospinal Fluid (CSF) of Patients In a Tertiary Healthcare Institution in Western Part of Nigeria","authors":"P. Adefuye, B. The aim of the study is to determine the prevalence of bacteria, which causes meningitis and their susceptibility pattern. This study, which was prospective and cross sectional involved patients with suspected symptoms of meningitis.1504 patients seen at the various facilities in University of Benin Teaching Hospital were involved in the study. CSF samples obtained from patients were incubated on chocolate, blood, and McConkey agar at 370c for 24hrs. Isolates were identified using the protocol of Cowan and Steel. Sensitivity test was done on Mueller-Hinton agar by the disc diffusion method of Bauer and Kirby using the Mcfalan method. The isolates varied in their susceptibility to the various antibiotics. Ofloxacin, ceftazidime amoxicilin clavulanate and gentamycin showed good antibiotic activity while cefuroxime, ceftriaoxone, cloxacillin, showed poor activity. Oxacillin showed the poorest activity. Despite the low infection rate, bacterial meningitis still remains a medical emergency and suggestions were advanced to control the occurrence. Health workers, particularly nurses could play an important role in the provision of tobacco cessation services in health care settings. This study sought to assess the baseline knowledge, attitude and practice of nurses in Lagos University Teaching Hospital (LUTH), Lagos, Nigeria, as part of a tobacco cessation training programme for health workers in the institution. A cross-sectional study was conducted among 108 nurses, who attended a one-day training workshop on the role of health workers in tobacco cessation in LUTH in August 2008, using a pre-tested self-administered questionnaire. Descriptive analysis was done using SPSS 16.0. Only 18.5% had good level of knowledge regarding tobacco use and cessation. The attitude of the nurses was relatively positive and 85.8% agreed that it was their responsibility to help their patients quit smoking and that patients who were smokers were likely to yield to counselling when offered by health workers. Only about half (51%) of the study participants routinely asked patients about their smoking status at first visit while 49% routinely offered some cessation advice. Majority (81%) were willing to spend at least five minutes of their time to assist their patients in quitting. Those who had received prior training on tobacco cessation had higher levels of knowledge but not necessarily better attitudes or practices compared with those who did not have prior training. The nurses surveyed have a poor level of knowledge and lack training on tobacco cessation. A significant proportion however had positive attitudes and were willing to assist their patients in quitting tobacco use. Regular training for nurses to equip them with the knowledge and skills to support and assist patients in quitting tobacco use are recommended. {"title":"Effect of Zinc supplementation on the Management of Acute Diarrhoea in Young Children in Jos, Nigeria: A Double-Blind Randomised Placebo Controlled Clinical Trial.","authors":"M. Episodes of acute diarrhoea tend to resolve within a few days in healthy children but persist longer in children with malnutrition, impaired cellular immunity and recurrent diarrhoea. Diarrhoea is very common in severe zinc deficient children and has been reported to respond to zinc supplementation as an adjunct to the combined use of oral rehydration therapy and early continued feeding. We study the effect of Zinc supplementation on the severity, duration and recurrence of acute diarrhoea in children in a hospital-based randomized placebo controlled clinical trial at JUTH in Jos. Seventy children 2-24 months of age who were reported to have passed at least 3 or more unformed stools in the previous 24 hours and have had diarrhoea for 7 days or less were randomized to receive Zinc (20mg/day n= 35) or placebo (n= 35) for 14 days. Each patient also had the low osmolar UNICEF-ORS at 75mls/kg. and the child's usual feeds. Weekly follow-up visits were conducted for 12 weeks after the diarrhea episode. Incidence and prevalence of diarrhea were compared between the groups. Neither the attending clinician nor the patient nor the data analyst was aware of who was on the Zinc or the Placebo. 57 children completed 14 days zinc supplementation, however only 54 children completed the three months post zinc supplementation follow-up. Zinc reduces the average number of watery stools by 37% and the mean duration of diarrhea by 39%. zxeZinc supplementation significantly reduced the risk of recurrence of acute diarrhoea by 38%. Supplementing the treatment of acute watery diarrhea with Zinc in just 2 cases will prevent recurrence of diarrhea in one of the children in the following 12 weeks. Fourteen days supplementation of Zinc in the treatment of acute diarrhea in children significantly reduced the average number of watery stools and duration of acute diarrhea. It also reduces the recurrence of diarrhea within three months of completion of supplementation. Zinc supplementation of acute diarrhea management in just two children will prevent diarrhea recurrence in the next three months in at least one child. In Nigeria, not much work has been documented on dental anxiety. The objectives of this study were to estimate the prevalence of dental anxiety and determine the reliability and validity of the Modified Dental Anxiety Scale (MDAS) in screening for dental anxiety among Nigerian adult dental patients. One hundred and ninety adult participants were invited to complete a sociodemographic questionnaire, Modified Dental Anxiety Scale and Hamilton Anxiety Rating Scale in order to determine the prevalence, internal consistency and concurrent validity of the Modified Dental Anxiety Scale. The mean score of the MDAS was 11.0 (SD = 6.1) and the scores ranged from 5-25. Of the total 190 participants, 60 (48.6%) scored above the cut-off score of 11. The internal consistency of the MDAS was 0.90 (95% CI = 0.94-0.97). The MDAS correlated well with HARS; the concurrent validity coefficient of 0.58 was obtained. (r = 0.90, p<0-0001). At normative cut-off score of 11.0, MDAS sensitivity was 0.8 and specificity 0.85. These psychometric findings showed that the MDAS has good internal consistency as well as good concurrent validity coefficient. Because of its reliability, validity, brevity and ease of administration, the MDAS is considered a useful instrument for measuring dental anxiety among Nigerians. Pre-tested, pre-coded questionnaires were used to interview subjects at the antenatal clinic. Most (95.8%) of the 288 respondents interviewed expressed the need for social companionship during labour. Forty-six (51.7%) multiparous respondents had delivered with a social companion during previous deliveries either at a private maternity unit or at their homes. Most (93.5%) of such respondents reported that, with a social companion during childbirth, they felt less pain, less fear and that they had a more satisfying experience of the childbirth process. Women that were 30 years of age or younger were significantly more likely to prefer a social companion during labour than older women (p = 0.0083). Educational status and parity did not influence women's perception of the need for social companionship during labour in the study population (p= 0.31 and p<0.01 respectively). Married women were significantly more likely to prefer social companionship during labour than sinlge women (p<0.005). Majority (46.0%) of such respondents preferred their husbands as companions during labour. Most women in our environment would want to have a social companion during labour and delivery. Such companionship during labour is associated with a more satisfying of the childbirth process by the women.
