This countertransferential need deserves to be regarded through the prism of the power inherent in the analyst's role. This allows us to transcend the limitations of dualistic metapsychology, and to make seminal contributions to traditional science. The new paradigm employs the concept of emergence, the potential for change in the evolutionary and clinical process. Emergence is described as originating with the Big Bang, but also is reflected at much higher levels, for example, biochemistry, or the capacity of the evolved mind to produce insights in psychotherapy. The constraints of dualistic theories are examined. A neuron-based view of change illustrates the evolution of traditional science as well as the neuron, itself. The new mind paradigm recognizes individual, familial, communitarian, and global reciprocal influences mediated by culture and illustrated by the extended mind and the democratic spirit. Thus both traditional and psychodynamic sciences are undergoing revolutionary changes in their common efforts to better understand the mechanisms of knowledge, relationship and consciousness. The boundaries of the self and the consultation suite are also expanded in this view. As this spread north, Shakespeare appeared and similarly invented and illustrated in his characters the private individual self, a concept not appreciated or recognized before the Renaissance. But as science arose and Western civilization began to decline, a corresponding disillusionment with “man” took place. The self began to be viewed as solely a social construction with no core except perhaps a genetic endowment. This was accompanied by a reduction in the concept of the human as a valuable and precious living being and was replaced by regarding the human as an object of control and exploitation. After the Second World War a movement in contemporary United States psychoanalysis gradually replaced the ideas of Freud and his emphasis on the “I” in the psychoanalytic process, with forms of relational therapy, assuming that the self was ab initio intersubjectively formed and could be altered fundamentally by focus on intersubjective processes. It is however the unmasterable subjective experiences such events provoke that injure the mind and ultimately the brain. Further, traumatic over-arousal may arise from inner affective deluge with minimal external stimulation. Affects that promote suicide when sufficiently intense are reviewed; we propose that suicidal crises are often marked by repetitions (flashbacks) of these affects as they were originally endured in past traumatic experiences. Further, recurrent overwhelming suicidal states may retraumatize patients (patients who survive suicide attempts survive attempted murders, albeit at their own hands). I would be hard-pressed to add any great new notions other than those proceeding from my experience as a creator of art and music in the style of the era mentioned in his article and discussions over many years with several prominent Medieval Art-Historians. There is danger in art-historical pronouncements of a later time that regard the panorama of earlier artistic currents—including, as Chessick does, the conception of the Self—in the light of subsequent developments. This is somewhat akin to using psychoanalytic interpretations to “understand” personages long dead from social settings very different from our own and certainly not available for therapeutic undertakings. It is a commonly held belief since Vasari in the 16th century that the culture of the Italian city-states was the initial rebirth of the true awareness of a Classical Aesthetic, an aesthetic placed into a state of hibernation by the Medieval Church and feudal decentralization. Yet, if we follow the paths of trade, education, and creativity, we fi nd great cross-currents from North to South and East to West. For example, the wool grown and sheared in England was taken via the traders and weavers of Bruges and Antwerp to be sold to the emerging class of highly wealthy Italians, often in the banking industry, who loaned money to the merchants and nobles of Europe to afford the very art they wanted. It is interesting to note that the fi nest manuscript in all Venice, possibly all Europe, was the Grimani Breviary, painted not in Italy but in Flanders. The famous “Tres Riches Heures” was painted for the brother of King Charles of France by the Limburg brothers from Dutch Flanders. The Hours of the Genoese Spinola family, for a time the most expensive manuscript ever sold, was painted by a Flemish team as well. Freud renewed and humanized psychiatry by enriching its static descriptive method with the new dynamic and interpretive discoveries of psychoanalysis. Freud's innovations, while developed in Europe, were integrated into psychiatry briefly by the Swiss School but mainly in the United States. After many years of fruitful collaboration psychiatry and psychoanalysis seemed to part company in the U.S. in the last few decades. Much of it dates back to the community mental health movement 50 years ago. The emphasis at that time was on deinstitutionalization, prevention, least restrictive options for care, and transformation of large public hospitals into multiservice regional centers. The current focus, in contrast, is on economy, local control and accountability, emphasizing, for example, manualized behavioral health techniques, pharmacotherapy, and case management by allied professionals. Psychodynamic psychiatry, particularly a psychodynamic orientation is uniquely helpful in complex, community-based clinical situations. Applicable psychodynamics include unconscious mental processes, psychological defenses, emotional relationships, a developmental perspective, disciplined use of the self for healing, transference, and communications in all forms as material for therapy. Literature and case reports illustrate the approach taken. These include examples of active listening, interprofessional collaboration, clinical continuity, appropriate boundaries, and thoughtful timing. Psychodynamic applications in community settings have passed the test of time and because of their validity could again become prevalent. Learning these different modalities at the same time can cause confusion in the areas of assessment, making a psychotherapeutic recommendation, and conducting a treatment. To investigate these issues, we presented a complex training case to three psychotherapy experts. Although they had somewhat different ideas about how to treat the patient, there was general consensus that adhering to a single conceptual formulation is key to treating individual patients in psychotherapy. Narratives of two residents who developed strong interest in psychodynamic psychotherapy and psychoanalytic training are presented to illustrate the important influences on their identities as psychodynamically oriented psychiatrists. Results from a recent survey of U.S. residents regarding psychodynamic psychiatry indicate that they value psychodynamic psychotherapy, want to incorporate psychotherapy into their careers as psychiatrists, and strongly endorse personal psychotherapy but view their psychodynamic skills as weak.
