{"title":"[Jawbone metastasis masquerading as dental pain].","authors":"Y Goldman, N Yarom","doi":"","DOIUrl":"","url":null,"abstract":"

Metastases to the oral cavity are rare. However, in 25% of cases, oral symptoms will be the first sign of metastatic disease. The incidence of jaws metastases is twice as high as the incidence of metastases to the soft tissues of the oral cavity. In some cases, jaws metastases can mimic dental or periodontal pain. We report a case of a 67 year old female who was referred to our clinic because of severe pain on her left posterior mandible which was not relieved by endodontic treatment of the first and second molar. She was diagnosed with breast cancer in 2005 and had been treated with surgery, chemotherapy and radiotherapy. Seven years later, lung metastases were found and she was treated with chemotherapy. Later on, brain metastases developed which had been treated with radiotherapy. On presentation, she complained of pain on the posterior left mandible which was accompanied by a burning sensation of the lower left lip and chin. CT scan revealed a soft tissue mass perforating the lingual and buccal plates of the posterior left mandible, which was compatible with a diagnosis of metastasis. Radiotherapy rapidly relieved the pain. Unfortunately, the patient passed away one month later. Dentists should be able to recognize the signs and symptoms associated with metastases to the jaws and should include it in the differential diagnosis, especially in patients with oncologic background.
","PeriodicalId":82453,"journal":{"name":"Refu'at ha-peh veha-shinayim (1993)","volume":"33 1","pages":"24-7, 61"},"PeriodicalIF":0.0,"publicationDate":"2016-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"34572274","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is expected to develop among the elderly, usually on the background of a weakened immune system. In those cases that the trigeminal branches are involved by zoster, unilateral mucosal and cutaneous vesiculo-ulcerative lesions will develop. Intense pain usually precedes the overt lesions of zoster, which sometimes might mimic acute pain of dental origin. Careful anamnesis and thorough clinical examination should lead to a correct diagnosis. There is wide agreement among clinicians and researchers that an extraction in young age is easier and safer, but it may result in overtreatment, which means an unnecessary extraction of teeth that might have been functional and healthy. In order to avoid such overtreatment the clinician is obliged to be able to predict accurately the chances of third molar eruption, years before they are due to erupt. Additionally, the clinician has to estimate the chances for future pathology or infection that will force the patient to extract the tooth in years to come. In order to do so the clinician has to meticulously examine the teeth both clinically (including caries assessment of neighboring teeth, periodontal pocket depth measurements etc.) and radiographically. The purpose of this literature review is to present a balanced approach towards the issue of early prophylactic extraction of mandibular third molar.
","PeriodicalId":82453,"journal":{"name":"Refu'at ha-peh veha-shinayim (1993)","volume":"33 1","pages":"45-51, 63"},"PeriodicalIF":0.0,"publicationDate":"2016-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"34573764","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

These are applied to improve the original image and to make the image more appealing visually. One possible means of enhancing digital radiographic image is sharpening. Oral Infections such as periodontitis may act as a distant reservoir of microbes, microbial products and inflammatory mediators. These might influence pregnancy and contribute to restriction of fetal growth and induction of early labor and PTB. Enamel formation of the primary teeth begins at 11-14 weeks of fetal life and is completed by the end of 3rd postnatal month. The initial phase consists of matrix formation, followed by calcification in utero. Since enamel is a stable structure, defects involving its matrix secretion and/or maturation of primary teeth can act as a permanent record of insults occurring pre- or perinatally. Any stressful event during pregnancy and birth may lead to metabolic changes in the formation of the enamel, resulting in clinically enamel defects. Severe infections occurring during amelogenesis may be associated with enamel hypoplasia. Babies born after maternal complications during pregnancy or babies who experience a traumatic birth must be considered to be at risk of developing Early Childhood Caries - ECC when exposed to excessive bottle nursing. Therefore oral healthcare should be kept during pregnancy.
","PeriodicalId":82453,"journal":{"name":"Refu'at ha-peh veha-shinayim (1993)","volume":"32 2","pages":"28-31, 38"},"PeriodicalIF":0.0,"publicationDate":"2015-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"33973242","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
