It bears a striking embryologic, histopathologic, and epidemiologic resemblance to another venereal pathology, cervical neoplasia in women. Although very similar, there is a significant difference between these two preventable processes. The incidence of cervical cancer has decreased, largely due to cancer screening programs; i.e., annual pelvic examination with Papanicolaou smear. To date, no ongoing screening program is in place for the early detection of anal neoplasia in this high-risk male population, accounting for the increasing incidence in anal carcinoma being seen. The following case report best illustrates how the lack of such a routine screening program can impact on a high-risk population; a population that is already besieged with the devastating effects of HIV infection. However, a majority of these studies have either concentrated on house staff physicians in regions which characteristically have a high-risk population base or have targeted primary care practitioners (family practice and internal medicine). This study focuses on exposure-prone physicians primarily engaged in invasive procedures (surgeons, OB/GYN, and emergency room physi cians) who practice within a \"moderate-risk\" population base (SMSA with a population of 475,000). Based on analysis of survey responses, the authors concluded that the majority of physicians engaged in exposure-prone invasive procedures are willing to treat PWA and PHIVs. Lehrman","doi":"10.1089/APC.1994.8.130","DOIUrl":"https://doi.org/10.1089/APC.1994.8.130","url":null,"abstract":"In this pilot study, a random sample of 50 nursing homes located in each of the five U.S. cities with the highest AIDS incidence were surveyed (total N = 250) to determine: (1) the frequency of admission inquiries by, and actual admissions of PWAs, (2) the frequency of adoption of formal AIDS admission policies, (3) the industry's concerns regarding provision of care in traditional nursing homes, and (4) the industry's preferred way of delivering long-term care (LTC) to PWAs. The majority of the facilities cited ability to meet special care needs, the costs of this care, and inadequate reimbursement as important admission policy issues. The majority indicated a preference for either special care units for AIDS or AIDS-specific skilled nursing facilities as the most appropriate setting for the provision of LTC to persons living with AIDS.
