In many cases, attempts to improve the quality of physician prescribing have been clumsy, often based on intuition. This article begins by describing the drug use process and the role of prescribing in that process. In the following section, we describe what is known about how physicians make drug choice decisions. This article discusses the complications associated with warfarin, unfractionated heparin, and low-molecular-weight heparin as well as the benefits of each. While a literature review turned up only limited data, the authors extrapolated from existing data recommendations for treatment during pregnancy, finding that oral warfarin should be replaced by heparin during pregnancy, especially from the 6th to the 12th week and near term. Currently, there are no universally accepted definitions for these terms. The authors of this study evaluated 44 articles which claimed to measure cost savings, cost avoidance, or cost reduction associated with clinical pharmacy services, and offered recommendations for when it would be appropriate to use these terms. Based on the results of our analysis, it is evident that controversy exists in the literature when determining if services result in cost avoidance or cost reduction. {"title":"Pharmaceutical care and health outcomes in community settings: a matched pair analysis of perceptions of hypertensive patients versus pharmacists.","authors":"D Franic,&nbsp;D S Pathak,&nbsp;D Mott","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Unlabelled: </strong>This study aimed to: (1) assess the level of pharmaceutical care delivered by pharmacists, and (2) compare patients' and pharmacists' health related quality of life (HRQOL) assessments for patients receiving antihypertensive therapy as an indicator of the delivery of pharmaceutical care.</p><p><strong>Design: </strong>An exploratory, cross-sectional study using matched pair survey instruments.</p><p><strong>Setting: </strong>Columbus, Ohio.</p><p><strong>Participants: </strong>Patients in a community setting prescribed antihypertensive medication and their pharmacists were selected for study participation.</p><p><strong>Interventions: </strong>Parallel surveys distributed to patients and their pharmacists.</p><p><strong>Main outcome measures: </strong>Level of pharmaceutical care delivered by pharmacists, and perceived patient viewpoints on: clinical outcomes (blood pressure control, and side effects due to antihypertensive medication) and humanistic outcomes (overall, emotional and physical HRQOL) by patients and pharmacists.</p><p><strong>Results: </strong>Analysis of 52 useable matching surveys of pharmacists and patients indicated that nearly all pharmacists counsel patients and two thirds of pharmacists counsel and monitor drug therapy. This is consistent with providing the minimal OBRA 90 requirements. A retrospective multicenter evaluation of aprotinin use was performed in high-risk coronary artery bypass graft patients treated either with aprotinin or according to usual-care to assess (1) differences in demographic and medical history characteristics, and (2) clinical and economic outcomes associated with their care. This study suggests that in many cases, the cost of aprotinin is offset by reductions in overall cost. Additional study is required to better understand this potential. A decision analysis model was constructed to compare the cost-effectiveness of the two antiemetics from the perspective of third party health care payers. Probability estimates of the model were derived from a reanalysis of data obtained from a clinical trial directly comparing the two antiemetics. Effectiveness was defined as the number of patients with no emetic episodes and no drug-related adverse events during the 3-day period after receiving chemotherapy. Cost-estimates were based on Average Wholesale Price for medications and national average for hospital services adjusted to 1996 U.S. dollars. Cost-effectiveness ratios for ondansetron and prochlorperazine were $261 and $268 per effectively treated patient, respectively. A series of sensitivity analyses were performed to identify the limits of generalizability of study results. Guidelines were developed with careful review of published literature and in cooperation with thought leaders in the institution. The primary focus of the guidelines was choice of medication and monitoring parameters and an educational campaign to ensure use of the guidelines. A prospective pharmacist intervention program was initiated for further enforcement of the guidelines. Barriers to effective communication between the pharmacist and the postmenopausal woman are examined, and suggestions are made to improve communication skills. Patient assessment and the development and implementation of care plans and continued patient monitoring are reviewed.
