HLH is one of the complications in dengue infection. A 69-year-old lady was treated for severe dengue with multi-organ dysfunction with superimposed pneumonia, requiring mechanical ventilation. However, persistent cytopenia despite blood transfusion without evidence of haemorrhage raised the suspicion of HLH. Further blood investigations revealed hypertriglyceridaemia, hypofibrinogenaemia and hyperferritinaemia. Bone marrow aspiration showed haemophagocytosis. Patient fulfilled the diagnostic criteria for HLH by HLH-2004 trial. Her HScore is 281, with the probability of having HLH is 99.9%. Patient’s condition improved after administration of intravenous immunoglobulin (IVIG) and intravenous dexamethasone in tapering doses. Early specific treatment of HLH with IVIG and/or corticosteroid is important but diagnosis is usually delayed due to nonspecific clinical findings and laboratory results. We report a 10-week-old male who had a scalp swelling for five days without any recent history of head injury. There was a soft, mobile, non-tender, transilluminating, fluctuant swelling over posterior parieto-occipital area and it was not limited by suture lines. Cranial ultrasonography showed a subaponeurotic collection at the posterior parieto-occipital area without definite communication with the intracranial space or the brain parenchyma. This infant was treated conservatively. The swelling resolved spontaneously after 4 weeks. The lack of awareness of some doctors in the Emergency Department about this condition may lead to unnecessary investigations, ward admission and intervention. Haemoglobin content of reticulocytes is useful to identify IDA at earlier stage. The objective of this study was to evaluate reticulocyte-haemoglobin equivalent (Ret-He) in diagnosing IDA in children and to compare it with other conventional methods. This prospective study was conducted on 120 paediatric patients aged 12 years and below, who attended Hospital Sultanah Aminah Johor Bahru, Malaysia with haemoglobin <12 g/dL. Ret-He and serum iron, ferritin and transferrin saturation were measured. Using a cut-off point of 20% for transferrin saturation, 81 out of 120 subjects (67.5%) were found as iron deficient. Based on the diagnosis of IDA, cut-off value for Ret-He using the Receiver Operating Characteristics (ROC) curve analysis was found as 22.65 pg. Ret-He showed a good sensitivity and specificity of 77.8% and 66.7%, respectively. As compared with Ret-He, serum ferritin showed a sensitivity of only 18.9%. However, a good specificity of 100% suggest it is useful for ruling in the disease but not suitable for screening. Transferrin saturation showed a good sensitivity and specificity, but it is biologically variable and not cost effective as a screening tool. Correlation study showed serum iron and transferrin saturation have significant positive correlation with Ret-He (r=0.415 to 0.518). However, there was no correlation between Ret-He and serum ferritin (r=0.051, p=0.578). Although indirect calorimetry (IC) is currently a gold standard for resting energy expenditure (REE) measurement, yet it is still not routinely used in the ICU. A total of 146 mechanically ventilated patients were randomised to receive enteral nutrition (EN) with energy targeted based on continuous indirect calorimetry (IC) measurements (IC group, n=73) or according to 25 kcal/kg/day (SWB group, n=73). Patient characteristics were equally distributed and the IC group showed lower mean measured REE (1668.1 + 231.7 vs 1512.0 + 177.1 kcal, p<0.001). Results also showed a significant deficiency in the daily (-148.8 + 105.1 vs. -4.99 + 44.0 kcal, p<0.001) and total cumulative energy balances (-1165.3 + 958.1 vs. 46.5 + 369.5 kcal, p<0.001) in the SWB group as compared to the IC group. From the Kaplan-Meier survival analysis, we found that ICU mortality was significantly lower in the IC group with better survival probability compared to the SWB group (log-rank test, p = 0.03). However, both groups showed comparable results in terms of ICU length of stay, duration of mechanical ventilation and incidence of feeding intolerance. The treatment of periodontal disease involved mechanical therapy, surgery, and administration of antibiotics. The high cost of the treatment and bacterial resistance to certain antibiotics regime has led to the search for the alternative treatment to overcome the disease at less cost and side effects. Natural products had been proven to be an important source in developing modern medicine to promote general health. Herbal plant extracts had been widely used in oral health particularly to prevent bacterial plaque adherence and dental caries. In periodontal disease, several studies and reviews have documented that herbal plant extracts had the potential to prevent and treat periodontitis through their active ingredients associated with antimicrobials, antiinflammatory, and anti-oxidant properties. The sources to obtain the herbs are readily available and economical to produce in the market. Some of the herbs had been marketed for public uses in the form of toothpaste and mouth rinses. The main means of financing healthcare include direct out-of-pocket (OOP) payment and government subsidisation in some countries, or public/private health insurance schemes, or a mix of all. Patient satisfaction is critical in ensuring the use of healthcare services, continuity of care and treatment adherence. In this study, we determined the satisfaction of type 2 diabetes mellitus (T2DM) patients regarding the healthcare services and payment methods at Universiti Kebangsaan Malaysia (UKM) Teaching Hospital, Malaysia. This cross-sectional study involved 313 T2DM patients aged ≥18 years who were included after clinical consultations. We used convenience sampling at the outpatient and inpatient medical centres of Hospital Canselor Tuanku Muhriz and UKM Specialist Centre. A survey consisting of sociodemographic, socioeconomic and payment method types as well as a validated patient satisfaction questionnaire scale were used. The mean age was 59.6 years (SD=13.151), 53.0% of the patients were female, 78.3% were Malay, 76.4% were uninsured, 39.6% were covered by government subsidies, while 36.7% paid OOP. Around 86% were generally satisfied with the overall services. Patients were most satisfied with technical quality (84%), communication skills (83%) and accessibility (80%), but satisfaction was lower in doctors’ service orientation, particularly the interpersonal manner (73%), financial aspect (73%) and time spent with the doctor (70%). Over 86% of patients were satisfied with healthcare services and payment methods; however, patients who paid OOP reported low satisfaction. With this study, it was aimed to evaluate the changes in certain healthy lifestyle behaviors, risky behaviors and depression levels of students enrolled at the different faculties of a public university in Istanbul during their university life. The data were obtained through a questionnaire of 65 questions, and the Beck Depression Inventory, which consisted of 21 questions. They were administered under supervision by providing preliminary information to the students. The mean body mass index, mean systolic blood pressure and mean diastolic blood pressure values of the students were 23.23 + 4.1, 108.97 + 15.21 and 66.98 + 10.08, respectively. The mean daily sleep time was 6.77 + 1.21 hours, the mean TV viewing time was 0.77 + 1.13 hours, and the mean time they spent with a smartphone, tablet or computer was 3.67 + 2.50 hours. Among the students, 37.07% were smokers, 26.12% consumed alcohol, and 4.77% used addictive substances. Of the students, 31.46% considered themselves at risk for depression. At the universities, there is a need for the implementation of health promotion programs focusing on promoting physical activity and healthy nutrition and reducing risky behaviors. Although it is very difficult to define the quality of life (QoL) precisely, it has a broad concept that affects global life satisfaction, which includes good health, adequate housing, employment, personal and family safety, interrelationships, education, and leisure pursuits. In this study, we aimed to discuss the life changes upon being diagnosed with breast cancer and their perception of social support. We also discussed the spirituality factor in patients’ coping mechanism with their health condition. This study was carried out using a purposive sampling method. The perception of social support, quality of life, and religiosity factor is discussed through the perspectives of nine breast cancer patients with an interview consisting of semi-structured questions. Basic interpretive qualitative methodology was applied to analyse the perception of social support and religiosity factor as a coping mechanism. The results showed that all the breast cancer patients reported good social support from family and friends except for one patient. Five themes were identified, namely patterns of life, supportive atmosphere, decision making and confidence, expectation, as well as psychological defences and spirituality. Seven out of nine patients reported that their religion helped them to cope with the disease by maintaining self-esteem, providing a sense of meaning and purpose, giving emotional comfort, and providing a sense of hope. The study is designed as descriptive research, and it was conducted with 369 medical school students and residents. A survey form was used in the research. A Chi-square test was used to compare categorical variables. In the study, 369 people were reached within the scope of the research. A total of 60.7% of the research participants were clinical medicine students (4th, 5th, 6th grade) and 39.3% were residents. A total of 42.0% of the participants of the study were male, 58.0% were women. It was found that 17.9% of the clinical medical students and 18.6% of the residents had sufficient knowledge of ionising radiation (p=0.002). A total of 87.0% of the participants in the study answered correctly that magnetic resonance imaging (MRI) does not contain radiation and 93.5% answered correctly that ultrasonography (USG) does not contain radiation. 74.8% of the participants stated that having knowledge about ionising radiation would contribute to “protecting sensitive groups from ionising radiation” in medical practice. This ratio is 77.2% in clinical medicine students and 71.0% in residents. The study found that knowledge levels of medical faculty students and residents about ionising radiation were insufficient.
