{"title":"Production problem encountered using a solvent drying plant.","authors":"A Gay, P Lines","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>All dirty surgical instruments received into the unit are processed through an Automatic Surgical Instrument Cleaning and Solvent Drying System. This system had been in continuous use since its installation in August 1983 until May 1986, without any significant production problem arising. During May 1986, and for a prolonged period afterwards, persistent separation of the surfactants contained in the Arklone W occurred. This separation manifested itself as a thick Cheeselike surface scum on the cooled solvent. At the onset the only recourse available in order to maintain the plants productive capacity was to replace the contaminated solvent with fresh following each occurrence. This constant replenishment obviously proved to be an extremely time consuming exercise. Much more emphasis is now placed on the measurement of quality of care. Changing the home environment requires considerable skill as the nurse really is the guest in the home. A sound body of knowledge is required in the areas of microbiology and immunology to enable infection and contamination risks to be assessed in the community. This knowledge may not be there in the mature nurse, trained many years ago. Resources are frequently limited and may be abused. Practice may primarily be based on ritual and tradition. Perhaps to some extent those working in the community are enjoying a false sense of security regarding the incidence of infection. The more recent concern relating to infection risk from HIV and Hepatitis B has made nurses question their actions, this can only be of benefit. The increasing quantity of clinical waste and toxic substances and incidence of infection hazard must surely cause those with concern for health in the community to examine practices previously unquestioned. {"title":"Loss of Crown Immunity: general implications for local authorities and district health authorities.","authors":"A Higgins","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In August 1984, 355 patients and 106 staff at the Stanley Royd Hospital in Wakefield were involved in an outbreak of food poisoning as a result of which 19 died. The inquiry following publicised, if not sensationalised the outbreak, found that there had been a complete failure to follow the basic rules of food hygiene practice which was not assisted by the condition and inadequacies of the kitchen, the environment in which the staff had to work or the tools and equipment they had to use.
