The most likely causes were respectively: a large blood exposed non-intimal surface (BENIS), unfavorable graft angle geometry inherent to connector designs, and tissue trauma. The insights gained by this analysis combined with our S2 connector exper-tise were included in the design criteria for our new connector. Once realized, the device was tested for the major usability criteria in laboratory using porcine and human ex vivo tissue. Results: Based on our previously developed and animal vali-dated S2 micro-stapling technology, we devised a novel coronary bypass connector having a very small BENIS and inflicting only minimal tissue trauma. Versatility was optimized by adopt-ing a side-to-side anastomosis configuration, allowing jump grafts, Y-grafts as well as end-to-side geometry conversion, also under limited access conditions. Finetuned staple param-eters allowed the use of both internal mammary as well as saphenous vein conduits. Ease-of-use was maximized by a stepwise, fully visually checkable deployment procedure. Laboratory tests showed excellent device efficacy in terms of consistently creating precise and widely patent anastomoses and freedom of leakage against high burst pressures in both porcine and human tissue. Soukiasian","doi":"10.1177/15569845221146369f","DOIUrl":"https://doi.org/10.1177/15569845221146369f","url":null,"abstract":"Objective: The aim of the cross-over study was to evaluate skill acquisition in lobectomy-naive surgical trainees completing a 4-week program to learn VATS lobectomy on a virtual reality surgical simulator (LapSim). Method(s): Lobectomynaive surgical trainees (year 1 and 2 postgraduation) were enrolled during the COVID pandemic from March to June 2021 for a 4-week course on basic VATS skills and right upper lobectomy. They were divided into 2 groups. Both groups completed an initial assessment, Group 1 completed the course first, then both groups completed a second assessment. Then Group 2 completed the course, and both groups completed a final assessment. Skill acquisition was assessed using instrument movement, procedure time, and blood loss for both the trained operation and an untrained operation (left lower lobectomy). Result(s): 16 trainees were enrolled, 10 completed the training program. There was no difference in baseline assessment. After Group 1 completed the training, they outperformed Group 2 in all metrics but this did not reach statistical significance. After training Group 2 at week 8, there was no longer difference in performance from Group 1. After completing the training program, the entire cohort showed a significant improvement in basic VATS tasks as well as lobectomies. There was statistically significant improvement in both right upper lobectomy instrument movement (P=0.002) and time (P=0.009) and left lower lobectomy time (P=0.047). Conclusion(s): This study showed that VATS simulation training on LapSim allowed junior trainees to learn advanced VATS resection during a pandemic and within 4 weeks. The acquired skills is transferrable to untrained operations. The objective is to determine the relationships between service system innovation and service quality and identify the key drivers of service system innovation in the telecommunication industry. The study adopts a quantitative research approach, using a survey questionnaire to collect data from 200 customers of three major telecommunication firms in Nigeria. Data analysis is conducted using descriptive statistics and partial least square – structural equation modelling (PLS-SEM). The findings reveal a significant positive relationship between service system innovation and service quality in the Nigerian telecommunication industry. The results also indicate that the key drivers of service system innovation in the industry include idea generation, service development and commercialization. The study concludes that telecommunication firms in Nigeria can enhance their service quality by investing in service system innovation and focusing on the key drivers identified in this study. This research contributes to the existing body of knowledge by providing empirical evidence of the relationship between service system innovation and service quality in the Nigerian telecommunication industry, as well as identifying the key drivers of service system innovation in the industry.
