A model is presented for collaborative practice between two advanced practice nursing roles--a nurse practitioner and a clinical nurse specialist/case manager. The model was developed from the clinical practice of the authors' work in a community health center. The Continuity of Care Prism Process was developed to meet the need to guide clients along the continuum of care and to achieve continuity of care. Advanced practice nurses are the \"expert\" clinicians in a position to care manage clients and meet the financial and quality constraints currently being placed on healthcare agencies. {"title":"Enteral and parenteral nutrition transitioning from hospital to home.","authors":"K Goff","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Enteral and parenteral nutrition therapies are routinely used in the acute care setting, rehabilitation facilities, and in home care for patients unable to ingest or absorb adequate nutrients orally. In many instances, a nutrition support service uses a team approach to administer and monitor this therapy in the hospital. The same nutrition support service may also arrange the transition from hospital to home for those patients who need to continue therapy long term. However, in settings where there is not a nutrition support service that coordinates the discharge, the case manager may be given this responsibility. Because of the complexity of reimbursement issues, particularly from Medicare, the monitoring needed after discharge, and the need for an experienced home care agency, the case manager will be better equipped to coordinate this discharge with knowledge about these issues. The case management system in this case study underscores the potential negative outcomes that can result from ignoring the holistic approach represented by an implemented case management system as a cost control mechanism. These negative outcomes not only include those expenditures of resources usually included when describing costs of care, but include those emotional costs of the system, patient, and care managers. A review of the historic literature reveals that the evolution of case management is a byproduct of creative healthcare efforts associated with different disciplines, including nursing, medicine, mental health, public health, and social work. As the emphasis on the delivery of cost-effective patient care increases, comparing outcomes across settings is desirable and essential. A key component to comparing how an organization rates with similar institutions is to identify commonly used measures.
