The focus will be around the consultation and decision-making process of prescribing and the difficulties faced when dealing with frail, uncommunicative patients. This article will explore and critique the evidence-base, local and national guidelines, and primary research around the pharmacokinetics and pharmacodynamics of nitrofurantoin, a commonly prescribed medication. Consideration of the legal, ethical and professional issues when prescribing in a non-medical capacity will also be sought, concluding with a review of the continuing professional development required to influence future prescribing decisions relating to the case study. Food- and water-borne diseases are still the most common health issue experienced by travellers, often causing diarrhoea. The vaccine-preventable diseases are poliomyelitis, hepatitis A, typhoid and cholera. Malaria is caused by the parasite Plasmodium, transmitted to humans by the bite of a female Anopheles mosquito, most active between dusk and dawn. Insect bite avoidance is crucial to prevent bites from infected mosquitoes causing infection. All travellers visiting malaria endemic areas are at risk of exposure. Conducting a pre-travel risk assessment and providing appropriate advice to the individual is essential. The malaria prevention guidance emphasises that effective bite prevention should be the first line of defence against malaria infection, even when chemoprophylaxis is used. Recommendations for antimalarials should be appropriate for the destination, tailored to the individual, and account for the risks and benefits to the traveller. Malaria is a preventable, but potentially fatal illness. Beverley Bostock provides an overview of what the latest publication means for general practice nurses The latest guidance from the Global Initiative for Obstructive Lung Disease (GOLD) on chronic obstructive pulmonary disease (COPD) will have an impact on the management of patients in general practice. The definition of COPD has been updated as well as the recommended approach to diagnosis. Updated treatment recommendations act as an important reminder for primary care clinicians to implement interventions that are relevant in the community setting and to refer on for more specialised interventions in secondary care as appropriate. The replacement of the ABCD assessment tool with the ABE approach underlines the significant impact that exacerbations can have in the management of COPD, independent of the symptom burden.
