The author argues that music therapy is essential for developing a holistic approach focusing on the developmental level of a child or young person, as well as being an inexpensive, non-pharmacological, non-invasive therapy, with significant physiological and psychological benefits. National Institute for Health and Clinical Excellence guidelines dictate that only one of these drugs should be given at a time because there is no evidence to suggest that simultaneous use is more effective. This article summarises a literature search of randomised controlled trials carried out to identify which, if either, of these drugs is faster at reducing a fever. In the studies examined, ibuprofen was found to be marginally more effective. However, the maximum dose of paracetamol was not administered. {"title":"Exploring the experiences of community-based children's nurses providing palliative care.","authors":"Sue Neilson, Joe Kai, Christine MacArthur, Sheila Greenfield","doi":"10.7748/paed2010.04.22.3.31.c7640","DOIUrl":"https://doi.org/10.7748/paed2010.04.22.3.31.c7640","url":null,"abstract":"<p><strong>Aim: </strong>To explore the experiences of community children's nurses (CCNs) and children's palliative care nurses (CPCNs) who provide end-stage palliative care to children with cancer in the family home.</p><p><strong>Method: </strong>A qualitative approach was adopted. One-to-one interviews and facilitated case discussions were undertaken with 30 community nurses who had provided palliative care to a child or young person with cancer. A grounded theory approach was used for data analysis.</p><p><strong>Findings: </strong>Because of the relative rarity of childhood cancer many CCNs and CPCNs engage infrequently in the palliative care of children or young people. This makes it difficult for them to develop and maintain knowledge and skills. To identify approaches that might reduce the adverse physical and psychological effects related to venepuncture.</p><p><strong>Methods: </strong>Questionnaire survey of 51 parents, 39 children and 72 junior doctors following venepuncture in the children. Telephone survey of 25 paediatric senior house officers on venous access training.</p><p><strong>Results: </strong>Parents and children assessed venepuncture as extremely distressing. Parents' assessments of the mean number of venepuncture attempts were markedly higher than those of the doctors surveyed. When carrying out venepuncture, doctors used therapeutic measures much less frequently than expected. Only seven of the 25 departments surveyed offered any training in venepuncture.</p><p><strong>Conclusion: </strong>Venepuncture is consistently one of the most distressing aspects of hospital admission for children.
