{"title":"[Total glossectomy with laryngectomy].","authors":"P Gehanno, C Guedon, B Baujat, J Depondt, B Barry","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Because local control of locally advanced cancer of base of tongue and larynx managed with combined chemotherapy and radiotherapy is poor, we proposed total glossectomy with total laryngectomy. Twenty-one patients underwent this operation over a period of 8 years. It was a salvage surgery for 15 patients, and an up-front surgery for the 6 others. Postoperative complications occurred in ten patients, and three patients died in the postoperative period. Mean total duration of hospitalisation was 40 days. Oral feeding could be achieved in 14 patients (mean delay: 46 days), but satisfactory oesophageal voice has never been obtained. We can conclude from the analysis of this series that total glossectomy with total laryngectomy is a heavy surgery, which leads to frequent complications, severe functional sequellae, and poor survival. However, control of pain can be obtained, that justifying this heavy procedure. We recommend that patients and relatives should be fully informed about the consequences of surgery before final decision.</p>","PeriodicalId":79321,"journal":{"name":"Bulletin du cancer. Isotransplanted AT17-mammary carcinomas on C3H-mice and osteosarcomas OTS-64 on Balb c-mice received 2 or 6<!--> <!-->Gy neutrons single dose. Before and at certain points of time after treatment the pO<sub>2</sub>-values were evaluated. Some tumors with initially low median pO<sub>2</sub>-values showed a short-lasting increase between 2 and 24<!--> <!-->hours after irradiation. In the tumors with relatively high median pO<sub>2</sub>-values before irradiation the pO<sub>2</sub> decreased to hypoxic range. A third group of tumors showed no significant changes after irradiation. None of the tumors stopped growth during the observation period.</p></div><div><p>Des recherches ont éré développées sur la tension en oxygène intratumorale après irradiation avec des neutrons de fission utilisant le « Eppendorf-pO<sub>2</sub>-Histograph<sup>R</sup> ». Des souris Balb c, iso-transplantées avec des ostéosarcomes OTS-64, avec des tumeurs mammaires ATI7 ou des souris CH3, iso-transplantées ont reçu une seule fraction de 2 ou 6<!--> <!-->Gy neutron. Avant et à différents intervalles de temps après traitement, la pO<sub>2</sub> a été évaluée. Pour certaines tumeurs à valeur moyenne de pO<sub>2</sub> initiale basse on note une augmentation de courte durée de la tension en oxygène entre 2 et 24<!--> <!-->heures après l’irradiation. Dans les tumeurs à pO<sub>2</sub> moyenne haute avant l’irradiation, la pO<sub>2</sub> dècroiît et se rapproche de l’hypoxie. Le troisième groupe de tumeurs ne présente pas de modification évidente après irradiation. Wilbur","doi":"10.1016/0924-4212(96)84911-5","DOIUrl":"10.1016/0924-4212(96)84911-5","url":null,"abstract":"<div><p>Fast neutron radiotherapy has proven to be an effective form of treatment in a selected subset of tumors (salivary gland tumors, sarcomas, and locally-advanced prostate cancer), but has not proven to be more beneficial than conventional photon irradiation for the majority of tumor types upon which it has been tested. Normal tissue tolerance limits preclude simply further escalating the neutron dose. Boron neutron capture (BNC) provides a way of selectively augmenting the radiation dose to the tumor. This process is described, and cell culture and animal model data reviewed. An irradiation configuration was developed where an enhancement of 2·10<sup>−3</sup> for l<!--> <!-->μg of <sup>10</sup>B per gram of tissue was achieved. This is similar to the enhancement achievable in the center of a 20<!--> <!-->×<!--> <!-->20 cm field envisioned for future applications such as metastases in the brain. A boron concentration of 50<!--> <!-->μg per gram of tumor tissue leads to a 10% increase in the delivered physical dose in this scenario. The first human test of BNC enhancement of a fast neutron radiotherapy beam using pharmacologically-acceptable doses of orally-administered, <sup>10</sup>B-enriched, <em>L-para</em>-boronophenylalanine is reported. An enhancement of tumor response was demonstrated for a melanoma skin nodule test system. Boron levels achieved in blood, skin, and tumors are presented. Future research plans are discussed.</p></div><div><p>La radiothérapie par neutrons rapides a fait la preuve de son efficacité dans le traitement de quelques tumeurs selectionnées (tumeurs des glandes salivaires, sarcomes et cancers localement évolués de la prostate). Elle ne présente pas d’avantage démontré par rapport aux rayons X dans la majorité des autres tumeurs évoluées. Les tissus sains en limitent souvent l’utilisation en ne permettant pas de donner les doses de neutrons nécessaires au contrôle local. La capture des neutrons par le bore (CNB) permet d’augmenter la dose à la tumeur de manière sélective. Ce processus est décrit et les résultats obtenus en culture cellulaire ou in vivo sur modèle animal sont revus. Une configuration d’irradiation a été développée dans laquelle la dose est augmentée de 2<!--> <!-->x<!--> <!-->10<sup>−3</sup> pour 1 μg de <sup>l0</sup>B par gramme de tissu. Ce résultat est comparable à celui obtenu au centre d’un champs 20<!--> <!-->×<!--> <!-->20 cm qui pourrait être utilisé en clinique pour des métastases cérébrales, par exemple. Dans ce cas, une concentration de 50<!--> <!-->μg par gramme de tissu produit une augmentation de dose de 10%. Le premier essai clinique a évalué l’administration orale d’une molécule de <em>l-para</em>-borophénylalanine enrichie en <sup>10</sup>B avant un traitement par neutrons rapides pour un mélanome malin. The technique has been successfully used to measure the gamma, fast neutron and the boron dose rates in the mixed radiation field from a <sup>252</sup>Cf source. The measured fast neutron and boron dose rates are in good agreement with the calculations and the measurements reported by other researchers. A systematic discrepancy is observed in the gamma dose rate measurements, with the present measurements being approximately 35% lower than those reported elsewhere.</p></div><div><p>Une technique de dosimetrie basée sur des compteurs proportionnels microdosimétriques a été développée pour la dosimetrie des faisceaux d’irradiation mixtes utilisés dans le traitement par capture des neutrons par le bore (BNCT). Cette technique a été utilisée avec succès pour mesurer les débits de doses de y de neutrons rapides et des particules secondaires à la capture dans un faisceau émis par une source de californium 252. Les débits de doses mesurés pour les neutrons rapides et les particules secondaires à la capture sont en accord avec les résultats calculés et les mesures réalisées par d’autres équipes.
