Superficial temporal artery pulse volume rates were made in a group of subjects diagnosed as having TMJ dysfunction and in a group of normal subjects. Bilateral plethysmographs were taken of five mandibular positions per subject. The graphs of the TMJ symptomatic group and of the normal asymptomatic group were evaluated by three non-dentists. The results indicated that the evaluators were able to separate the two groups with high reliability based solely on the readings of the vascular graphs. The author has successfully utilized this explanation for a number of years, and presents it when additional information is requested by insurance companies regarding difficult TMJ cases. Many times the patient's chronic “idiopathic” headache and facial pain have been misdiagnosed or undiagnosed for long periods, which affects the possibility of the treated patient's recovering fully. The normal anatomy and motion of the joint will be discussed and followed by a brief description of the arthrographic technique. Examples of normal arthrograms and common arthrographic abnormalities will be presented. These common abnormalities include reducible and irreducible anterior dislocation of the articular disc and perforations of the posterior discal ligament. The procedure is indicated in many patients with internal derangements, following splint therapy. Lingual buttons are bonded to the properly prepared buccal surfaces of the posterior teeth. The author then uses elastics for eruption force and composite filling material hardened by ultraviolet light to hold other teeth while the selected eruption is accomplished. A strong controversy exists between the proponents of the myofascial pain dysfunction syndrome and the occlusal dysfunction syndrome, yet neither syndrome has been scientifically proven. To the contrary, problems such as internal derangement, degenerative arthritis, and other forms of arthritic diseases appear to be significant causes of CMA dysfunction and pain.It is recommended that curriculum be established to redefine and classify craniomandibular problems in a logical and scientific manner. It is suggested that the general categories include developmental abnormalities, diseases, and dysfunctions. Physical therapy in collaboration with dentistry provides an increasingly successful approach to longer lasting treatment. A correlation between Class II occlusion and forward head posture provides further evidence that the team approach is essential. Many TMJ headaches and referred pains of the neck and shoulders are caused by compression of the cervical joints. The proper orientation of four planes: the vertical plane, the bipupilar line, the plane of the otic system, and the occlusal plane are necessary for case success. Treatment involves the proper body mechanics, overcoming parafunctional oral habits, and instruction to restore mobility of the spine.
