{"title":"Intraocular hemorrhage in a patient with hemophilia.","authors":"H Kobayashi,&nbsp;Y Honda","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A 24-year-old man with severe classic hemophilia (factor VIII level less than 1%) had blunt trauma to his left eye that produced total hyphema with elevated intraocular pressure. Despite attaining sufficient hemostasis with factor VIII replacement, the anterior chamber remained completely filled with blood, which led to corneal blood-staining. Total hyphema was removed under management of replacement treatment. This consisted of sufficient cryoprecipitates to increase the calculated factor VIII level to 100%. For the 7-day post-operative period, the factor VIII level was above 15%. His surgical and post-operative course was uneventful. Blood-staining of the cornea was gradually reduced. Five months after surgery, he spontaneously had moderate hyphema with elevation of intraocular pressure. The blood in the anterior chamber was absorbed with replacement therapy. The present study compared corneal sulfation in vivo and in vitro, with sulfation in cartilage of rats undergoing total food deprivation for 48 hours and refeeding for periods up to 28 days. Sulfate incorporation by cartilage decreased both in vivo and in vitro during the fast and recovered to normal values by 7 days of refeeding. Corneal sulfation increased in vivo during the fast. After 7 days of refeeding corneal sulfation in vivo dropped below the control value and remained below the control level. At later recovery periods corneal sulfation in vivo was normal. Corneal sulfation in vitro decreased during the fast. Normal cartilage showed a decrease in sulfate incorporation in vivo and in vitro with increasing age. Corneal sulfation in vitro followed this pattern, but corneal sulfation in vivo did not. The patient suffered further of general arthrosis and osteopoikilosis in the proximal epiphysis of the femur, demonstrable by X-ray examination. The authors compare the syndrome of their patient with the artificially induced calcification syndrome of rats, which was developed by H. Selye et al, after the administration of isoproterenol, 5-hydroxitriptamin, and dihydrotachysterol to the animals. All three compounds inducing the artificial calcification syndrome, or similar ones, are also normally present in the organism. {"title":"ERG changes after an injection of corticosteroid into the vitreous.","authors":"K Koizumi,&nbsp;Y Honda","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>We injected a high dose (5 mg) of pure betamethasone acetate (a water-insoluble corticosteroid) into the vitreous space of aphakic rabbit eyes after and without closed vitrectomy, and followed ERG b-wave amplitude changes over 36 days. The ERG b-wave amplitude decreased immediately after vitrectomy, but recovered in one day. Administration of betamethasone increased the ERG amplitude in both eyes to abnormally high levels. The highest amplitude appeared on the 3-4th day and this enhancement continued for 7-20 days. This radiobiological procedure results in the selective inhibition of growth-hormone (GH) release. A series of 107 eyes were analyzed after division into 2 categories A and B (A: progressing background DR, n = 53, B: proliferative DR, n = 54). All patients treated by laser photocoagulation before and/or after stereo-GIHF were excluded from the present study; in contrast complications that impair visual acuity (VA), mainly vitreous hemorrhages, diabetic maculopathy and cataracts, were not discarded. In groups A and B, mean VA was stable showing no significant differences before and 5 years after treatment. In both groups microaneurysms are improved or stable in all cases. Hard exudates were improved in 66% of cases. In group B preretinal new-vessels and neovascularisation on the disc are improved or stable in 90% of cases. The same percentage is observed for extramacular and macular transudations 3 years after Stereo-GIHF. The present data strongly suggest that the selective suppression of GH improves the prognosis of DR. Previous studies of tryptophan plasma and serum metabolism are conflicting. In this study free and bound TRP plasma levels were measured in patients with senile cataract and in controls after an oral load of L-TRP (20 mg/kg b.w.). In all patients with poor fusional amplitudes the reading mechanism could be improved with orthoptic exercises designed to augment the fusional amplitudes.
