{"title":"[Improved care for the aphasic patient: introduction to the central theme of aphasia].","authors":"E Dekker","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The Dutch Hart Foundation has as one of its major goals the furthering of revalidation of hart patients as well as patients with cerebrovascular illnesses. Since many of these latter patients suffer from aphasic disturbances, the CVD-committee of the above mentioned foundation has, in co-operation with the Dutch Society for Neurology, investigated the possibilities of improving the treatment of these disturbances. A systematic improvement of this treatment depends on the ability to measure it. Therefore, the foundation has invited a number of prominent researchers in this area for a workshop in August 1979 posing the question whether and if so how the severity of aphasia can be measured. {"title":"[Psycholinguistic aspects of aphasia diagnosis and therapy].","authors":"R S Prins","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A central goal of psycholinguistic research on the diagnosis and therapy of aphasic patients consists of the construction of a series of language tasks which a. can provide guidelines for the development of systematic therapy programs and b. are suited to the evaluation of the recovery process in aphasia, in particular the effectiveness of speech therapy. Since current aphasia tests fall short on both points, a number of requirements are discussed which an aphasia test should minimally meet in order to satisfy the above objectives. Factor analyses demonstrate three factors that can be interpreted clearly as spontaneous speech, production and comprehension of speech. This structure is very stable during a follow-up of one year. The tests and the production, comprehension and severity scores derived from them have high indices of reliability. The research has provided a number of interesting data. There are, however, some serious methodological problems, which have not been adequately handled by the investigators. Especially, the very abstract nature of the concepts does run up against serious objections. Also the nature of the research problem has been discussed. It is suggested that another frame of reference could be more fruitful, namely the discrepancy between the development of the aged population in the last fifty years on the one hand and the organizational structure of the government on the other hand. Possibly there is a large 'cultural lag'. {"title":"[Invalidity and aphasia].","authors":"C Leering","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Aphasia may be conceived as a disturbance in one of the functional capacities. Most functional capacities may be 'measured' by performance scales (Katz, Barthel Index, Kenny Numerical Scale). These scales do measure more the 'amount' or 'need' of help than the disturbance itself. So, they measure in fact economic variables instead of medical ones. In 'Regina Pacis' we developed medical scales for the ADL and performance in ambulation. The question is raised whether in certain conditions, they might serve as parameters for the improvement of speech in aphasia. From this starting point these conditions are reviewed and it is thought that a common denominator for both disorders may be found: the ideamotoric disturbance, caused by the CVA.
