We report a case of portal vein thrombosis after laparoscopic colectomy for sigmoid colon cancer with an isolated liver metastasis. A 72-year-old man underwent laparoscopic sigmoidectomy for an advanced sigmoid colon cancer with a single liver metastasis. Pathological examination revealed a moderately differentiated adenocarcinoma, pSE, pN1, ly1, v2. The postoperative course was good. Two weeks after the laparoscopic operation, we performed an abdomen CT to evaluate liver metastases. CT revealed slightly greater liver metastasis than preoperation, and a filling defect of 5 mm in the right portal vein. There were no symptoms. Resection of the right hepatic lobe was performed. Pathological examination revealed isolated liver metastasis without vascular invasion and a thrombosis in the right portal vein. The patient showed no recurrence and no thrombus for 6 months after the radi-cal surgery. In this case, we accidentally detected the asymptomatic portal vein thrombus which occurred after laparoscopic colectomy. An enhanced CT scan revealed a suspected 3-cm-large aneurysm with contrast enhancement in the early phase in the right mesentery. Transcatheter arterial emboliza-tion was performed and the patient was discharged after follow-up. Six months later, the patient visited the emergency room due to abdominal pain, and enhanced CT revealed a 5-cm-large mass with partial contrast effect in the right mesentery. Suspecting re-rupture of the aneurysm, an ileocecal resection was performed. Operative findings revealed a tumor in the right mesentery, so the tumor was resected. Pathological examination revealed that the tumor was mesenteric malignant lymphoma. Pathological findings revealed mu-cinous adenocarcinoma ( T4N0M0 ) with R1 resection. Six months after surgery, local recurrence developed and he was re-ferred to our hospital. CT findings showed that the recurrent tumor, 30 mm in diameter, was adjacent to the right side of the prostate and the ventral side of the VRAM flap, however, there was no finding of invasion of the feeding vessels. Total pelvic exenteration ( TPE ) with preservation of the VRAM flap was successfully performed. He was discharged on the 27th day after surgery and remains alive without recurrence 1 year after surgery. The VRAM flap has been widely used in perineal reconstruction after extended pelvic surgery. In the case of local recurrence thereafter, redo surgery may be complicated, especially when the flap should be preserved.
