[Early and late complications of radius fractures in the classical location]. 【经典部位桡骨骨折的早期和晚期并发症】。

{"title":"[Early and late complications of radius fractures in the classical location].","authors":"J Meine","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The early and late complications of fractures of the distal end of the radius are evaluated from the records of 853 cases treated in a private surgical practice. Whilst the morbidity of the 123 fractures in children and adolescents is practically negligible, the incidence of complications in the series of 730 fractures in adults amounts to 28%. The complications in adults are analysed and separated into 8 categories: early dislocations, healing in malposition, compressive neuropathies, reflex dystrophy syndromes, persistent pains, rupture of the extensor pollicis longus tendon, stenosing tenosynovitis, and other complications. {"title":"[Autologous blood transfusion in surgical interventions].","authors":"W Hackenbruch","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>By the autologous blood-transfusion we understand the dose of own blood in contrary to strange blood. The advantageous of the method are well-known. But nevertheless the own-transfusion is rarely realized. By the immune infirmity AIDS this method is of pressing importance at the present time. In orthopaedics and the traumatology of the movement mechanism especially selected operations and delayed emergency-operations are qualified for the autologous blood-transfusion. Regarding a ten years positive experience we introduce a simple and safe method which is also commonly used in the regional hospital in Langenthal since summer 1986. It is a combination of a pre-operative own-blood-transfusion and the \"isovolumic\" actual blood-dilution. 158 patients have been analysed which have been treated by this method. The most frequently operation has been the total hip replacement 85% of the analysed persons could be operated without any strange blood and totally 426 times autologous blood donation could be received within 26 months. The follow-up shows that conservative methods suffer from high rate of axial deviation, particularly antecurvature, while operative treatment has the negative effect of a higher, incidence of persistent leg lengthening and of re-fracture. A three-dimensional correction at the metaphyseal level is technically feasible. This not only restores the normal orientation of the articular surface thus reestablishing normal load distribution but also the radial length up to 10 mm which is a prerequisite to prevent ulno-carpal impingement. Painful limitation of forearm rotation due to degenerative changes in the distal radioulnar joint may be treated with Bowers hemiresection arthroplasty. If the radioulnar index should be greater than 10-12 mm an additional shortening osteotomy of the ulna may become necessary. The system of Grosse and Kempf was utilized. The fractures were produced by a variety of mechanisms of injury, of which 70% were caused by traffic accidents. Associated injuries to other organ systems were sustained by 44% of the patients, overall there were 13% considered to have multiple trauma. 21% of the fractures were open. 21% required stable fixation, 17% were dynamized. The remaining 79% fractures were treated with dynamic nailings. A bridging callus was observed after an average of 6 weeks, and cortical bridging at 18 weeks following nailing. A posttraumatic limb length difference was present in 70% after femoral fractures and in 40% following fractures of the lower leg. In the control group of 822 examined persons we found an idiopathic difference of the length of the limb in 25%. Following femoral fractures the limb length difference was in average 10 mm. However following fractures of the tibia and fibula it was 7 mm. There is no evidence that an established difference in length will improve in course of growth. Factors influencing the alteration of length are discussed. It could be shown that there is no primary therapeutic way of influencing the alteration of length and that the so called prophylactic \"shortening\" at time of consolidation cannot prevent future lengthening. Also we could prove that rotational deformities of the femur show a good prognosis regarding spontaneous correction. Deformities of the lower leg however do not show spontaneous correction and ought to be corrected primarily. The incidence and degree of future alteration of length of the lower limb can only be influenced indirectly by early definite stabilisation of the fracture without leaving an axial deformity behind. For stable non-dislocated fractures the plaster of cast treatment is sufficient. However for unstable, dislocated (displaced) fractures which need anaesthesia for primary treatment, the unilateral external fixator is proposed as the method of treatment. Beside the recently recommended external fixator, the support plate fixation offers a helpful alternative to treatment. Between 1980 and 1986, 30 wrists in 29 patients with intra-articular fractures of the distal radius were stabilized with a buttress plate an the Kantonsspital Chur, Switzerland. The mean follow-up-time was 15 months. These follow-ups showed that the buttress plate in treatment of complicated intra-articular fractures allows a satisfactory reduction and stabilization with restoration of the articular congruity and the possibility for early active assisted motion.
