{"title":"Growth disturbance following metaphyseal bending fractures of the proximal tibia--an experimental study in the mini pig.","authors":"P Frey","doi":"10.1055/s-2008-1042603","DOIUrl":"https://doi.org/10.1055/s-2008-1042603","url":null,"abstract":"<p><p>Based on our clinical experience, we postulate that the unilateral \"posttraumatic genu valgum\" develops on the grounds of an unreduced primary valgus deformity. This induces a disturbance of the consolidation on the medial aspect of the fracture. Subsequently, a partial medial stimulation of the epiphyseal plate develops resulting in a secondary valgus deformity. We succeeded in proving experimentally this postulate using the Mini Pig as an experimental model. We investigated 33 tibiae of 17 Mini Pigs. With radiological follow-up studies we were able to show that the operatively created primary valgus deformity induces an increased valgus deformity of functional significance. This is a model of the unreduced fracture in men. However, the transection of the pes anserinus and the periost per se did not provoke a significant valgus formation. Based on these experimental results and our clinical findings we postulate the following treatment for the metaphyseal bending fractures of the proximal tibia in men: 1. Accurate, usually conservative, reduction of any primary valgus malposition of the fracture. 2. Retention of the corrected position of the tibia in a plaster cast. 3. A 6-week-old female was brought to our hospital because of an upper abdominal mass. A plain x-ray demonstrated a mass of structured bones that suggested a teratoma or fetus in fetu. At operation a 20 cm mass was found in the mesocolon covered by a membrane. The mass corresponded to a fetus in fetu connected by an umbilical-like cord to the host in the depth of the radix mesocoli. The fetus in fetu weighed 150 g and had severe malformations, such as internal hydrocephalus, a left side anotia, a descended right ear, anophthalmia, a right-sided amelia, a micromelia on the left side with 4 fingers, a sympus with 5 and 7 toes on the left and right side, respectively. Further intestinal resection in an already short gut was avoided by using Cisapride, a new intestinal prokinetic agent (1 mg/kg/d in 4 doses, orally), which dramatically improved the symptoms and allowed weight gain and intestinal adaptation. After 6 months, Cisapride was withdrawn. Intussusception could generally be diagnosed by ultrasonography in addition to the history and typical clinical signs without using a barium enema. The success of reduction could also be safely evaluated from the disappearance of both the characteristic sonographic target sign and the abdominal palpable mass, and improvement in the general condition 2 or 3 hours after completing this procedure. This simple, cheap and clean reduction method was applied to 127 patients under strict indications and sedation with diazepam. The successful reduction rate was high (90.6%) and equal to that by hydrostatic reduction. {"title":"[Precipitating factors for shunt insufficiency in post-hemorrhagic hydrocephalus in the premature infant].","authors":"B Resch, W Müller, R Oberbauer","doi":"10.1055/s-2008-1042581","DOIUrl":"https://doi.org/10.1055/s-2008-1042581","url":null,"abstract":"<p><p>Between January 1984 and March 1989 twenty-seven low birth weight infants (mean birth weight 1351 gm, mean gestational age 30 weeks) required shunts after development of a posthaemorrhagic hydrocephalus. Revision of the shunt occurred in 78% of the patients with a range of 1 to 11 revisions. Obstruction of the ventricular catheter was the main cause of mechanical complications that occurred in 75%. Preterm infants weighing less than 1000 gm revealed an enormous infection rate of 71%. Initial shunt placement in the first 8 weeks of life was more likely to need shunt revision (94%) than that placed at older age (44%). There was no difference between the type of shunt and percentage of shunt revision, but the Heyer-Schulte system in comparison with the Codman Uni shunt was more likely to have mechanical complications. Infants with Grade IV haemorrhage required the same percentage of revisions as those with Grade III haemorrhage. There was no association between preoperative therapy and the need for shunt revision. Our present scheme of management, based upon preliminary video-urodynamic assessment, is outlined. In the years 1984-1988 we treated 156 children suffering from neuropathic bladder. In 5 per cent of cases the upper renal tracts have deteriorated on treatment; in patients presenting with upper tract dilatation, improvement has been obtained in 68 per cent. A quarter of the patients were considered too generally disabled to achieve appliance-free continence; these have been managed by penile appliance, indwelling urethral catheter or, occasionally, urinary diversion. For three-quarters of the patients, most ambulant, appliance-free continence was the goal; 68 per cent have been managed nonsurgically and 32 per cent surgically.
