It is found that the release of fibrinopeptide A is fast as compared to that of fibrinopeptide B. This process is modulated by ATP in a complex fashion. In fact, the release of fibrinopeptide A is first enhanced at low (less than 1 mM) ATP concentrations and then progressively inhibited at ATP concentrations greater than 1 mM. In this work the cardiocirculatory effects of the isometric contraction of masticatory muscles were compared with those evoked by the handgrip exercise. Non invasive and, in one case, invasive (aortic catheterism) monitoring of arterial blood pressure was performed. Computer interfaced to instrumental equipments allows the monitoring of biomedical signals and their transmission at distance. Finally, the possibilities of computer-aided instruction are presented. {"title":"[Tissue-type plasminogen activator].","authors":"F M Turazza,&nbsp;M Pogna","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Tissue-type plasminogen activator (t-PA) is a serine protease that converts a zymogen plasminogen into an active serine protease, namely, plasmin. Plasmin is the proteolytic enzyme that degrades fibrin. In the absence of fibrin, e.g., in circulating plasma, t-PA activates plasminogen at a very slow rate. However, when fibrin is present, this activity is enhanced two to three orders of magnitude. As a consequence of these kinetic characteristics, plasmin is predominantly generated on the fibrin surface. This in turn results in a relative sparing of circulating fibrinogen and other plasma proteins to plasmin--mediated degradation. Following the demonstration of the potential of natural t-PA as a thrombolytic agent, an intensive effort was launched to enhance its production by recombinant DNA technology. The pharmacological action and the clinical efficacy of t-PA has been tested by several Authors in the treatment of acute myocardial infarction (AMI), and more recently, of pulmonary embolism, a condition for which this drug seems to be very promising: from this point of view this short article provides evidence that the various thrombolytic agents are of equal ability in mediating the rapid lysis of a coronary thrombus after i.v. administration when given appropriately and at the proper time; clinical experience provides little support for the contention of the superiority of t-PA over other thrombolytic agents, particularly for coronary thrombolysis. In all cases hypersideraemia developed within 24 hours: iron mean value increased from 70.5 micrograms/dl to 237.4 micrograms/dl (p less than 0.001). Return to basal levels was achieved after 8-9 days. Data have been collected suggesting a relationship between viral infections and HD. HD patients with evidence of Epstein-Barr virus genomes in their affected tissues have been recently reported. Human Herpes Virus six (HHV-6) is a newly isolated virus, derived from patients with lymphoproliferative disorders. In order to investigate the possible role of this virus in the pathogenesis of HD we looked for a specific segment of HHV-6 genome by means of the polymerase chain reaction (PCR) in tissue samples obtained from peripheral blood and lymphnodes in HD patients: one clearly positive case has been identified. This result is the first indication of HHV-6 sequences associated with a case of HD and raises the possibility that this virus might be involved in the pathogenesis of this lymphoproliferative disorder. A score of 1 was assigned to each of the following features: bone marrow plasma cells more than 30%, hemoglobinemia less than 11 g/dl, lytic bone lesions of degrees 2-3, presence of Bence Jones proteinuria and serum beta 2-microglobulin levels higher than 8.0 micrograms/ml. Therefore, the score for each patient ranged from 0 to 5, corresponding to six risk classes: score 0 = class I; score 1 = class II; score 2 = class III; score 3 = class IV; score 4 = class V; score 5 = class VI. Since no differences in mean survivals and in survival curves were found between classes I and II, between classes III and IV and between classes V and VI, three stages could be devised: stage A (good prognosis) corresponding to classes I and II; stage B (intermediate prognosis) corresponding to classes III and IV; stage C (poor prognosis) corresponding to classes V and VI. Significant differences were found among the three stages regarding mean survivals, survival curves, and response to treatment. This scoring staging system is very simple in its formulation; only five routine parameters and no calculations are necessary for obtaining a score and consequently a stage for each patient.
