According to a well documented retrospective study we describe the value of the shoulder arthroscopy and its correlation to MRI and arthro-CT findings. MRI represents nowadays the best way of exploring rotator cuff lesions and of investigating an impingement syndrome. Associated with a contrast medium its ability to clear up an anterior or posterior shoulder instability is now superior to the arthro-CT's. The arthroscopy allows the dynamic exploration of the joint and can be used not only as a diagnostic but also as a therapeutic procedure.
","PeriodicalId":77470,"journal":{"name":"Zeitschrift fur Unfallchirurgie und Versicherungsmedizin : offizielles Organ der Schweizerischen Gesellschaft fur Unfallmedizin und Berufskrankheiten = Revue de traumatologie et d'assicurologie : organe officiel de la Societe suisse de ...","volume":"86 1","pages":"4-17"},"PeriodicalIF":0.0,"publicationDate":"1993-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19344635","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
{"title":"[Alpine skiing--accident--and equipment studies].","authors":"W J Ziegler, P Matter","doi":"","DOIUrl":"","url":null,"abstract":"

According to medical statistics over the last two decades, skiing is still associated with a certain risk of accident and injury. A review of studies carried out in Davos on alpine skiing accidents covering two decades aimed at assessing the effect of safety improvements in ski bindings combined with ski shoes and skis in view of injury statistics. Five comprehensive research studies included almost 10,000 injured and non-injured skiers and experience with nearly 29,000 injured skiers treated the Davos hospital. Firstly, the causes and preceptating factors of skiing accidents were determined and the role of binding release tests assessed. Then the effect of the skier on the accident was investigated. The function of ski binding adjustments was also studied. In a fourth step comprehensive data on multicentric research project were obtained. With the growing popularity of safety bindings in the 1970's and the results of research projects, the frequency of skiing accidents seemed to diminish or at least stagnate. The annual statistics from 1974 to 93 show a reduction in the frequency of tibial fractures and simultaneous rapid increase in the frequency of injuries of the knee joint, the upper limbs and the trunk. The situation demonstrate that measurements to control the accident rate cannot be based only on accident statistics and technical developments, but must rely on further studies of the mechanism of the fall and analyses of the long-term consequences of injuries.
","PeriodicalId":77470,"journal":{"name":"Zeitschrift fur Unfallchirurgie und Versicherungsmedizin : offizielles Organ der Schweizerischen Gesellschaft fur Unfallmedizin und Berufskrankheiten = Revue de traumatologie et d'assicurologie : organe officiel de la Societe suisse de ...","volume":"Suppl 1 ","pages":"68-81"},"PeriodicalIF":0.0,"publicationDate":"1993-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19115407","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Both the effect of the bandage, i.e. the soothing and de-swelling effect, the function improvement, therapy progress and success were monitored and evaluated. The rest was designed as a short-term study with standardised scoring. After the falsification principle, the therapeutic effect was confirmed if a positive effect was shown during this time interval. Twenty-six patients between the age 26 and 65 were tested, 16 males and 10 females. In 18 cases the right arm was treated and in 8 cases the left arm. 17 cases of epicondylitis radialis humeri and 5 cases of epicondylitis ulnaris humeri were found. {"title":"[Overuse damage in modern cross-country skiing].","authors":"B Hintermann","doi":"","DOIUrl":"","url":null,"abstract":"

The skating technique in cross-country skiing has changed the demands on the human body. The purpose of this study was to determine the stress on the locomotor system of young cross-country skiers, and whether overuse injuries result from this sport activity. 51 young cross-country skiers of the Swiss national team were interviewed and examined for problems and overuse injuries of the skeleton. The investigations were repeated yearly during a follow-up time of 3 years. Most of the painful irritations and most overuse injuries were localized in the femoropatellar joint and in the distal Achilles tendon. Orthotics were made in 36 cases (71%). Individual adaptations of training programs, as well as special physical exercises were proposed. The frequency of pain limiting the sport activity was reduced from 62.7% to 21.9% within 3 years. The amount of overuse injuries had also decreased from 66.7% to 32.0%. This may signalize the reversibility of overuse injuries following normalization of loading stress although sport activity continues. In particular, no relation was observed between the frequency of overuse injuries and the new skating technique. However, it was shown that skating and jogging may stress the same structures and thus lead to excessive pronation.
","PeriodicalId":77470,"journal":{"name":"Zeitschrift fur Unfallchirurgie und Versicherungsmedizin : offizielles Organ der Schweizerischen Gesellschaft fur Unfallmedizin und Berufskrankheiten = Revue de traumatologie et d'assicurologie : organe officiel de la Societe suisse de ...","volume":"Suppl 1 ","pages":"274-85"},"PeriodicalIF":0.0,"publicationDate":"1993-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19115506","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Localized hematomas should be evacuated;--fractures must be definitively stabilized as soon as possible. Therefore, internal fixation should be performed primarily whenever possible;--primary skin grafting of all skin defects to insure early wound closure. {"title":"[Stress on the femoropatellar joint in downhill walking--a biomechanical study].","authors":"M Kuster, G A Wood, S Sakurai, G Blatter","doi":"","DOIUrl":"","url":null,"abstract":"

Kinematic and kinetic data were collected of 6 male and 6 female healthy subjects. For downhill walking a dismountable slope of 6 m length and a grade of 19.3% was built. Planar net joint moments and mechanical power at the knee joint were calculated for the sagittal view using force platform records based on standard inverse dynamics procedures. On the basis of a two dimensional knee model the patello femoral joint compressive forces were calculated. While the maximal extension moment for level walking was only 1.2 +/- 0.5 Nm/kg bw at a knee flexion angle of 20 degrees it was 2.6 +/- 0.6 Nm/kg bw at an angle of 40 degrees for downhill walking. This increased moment and knee flexion angle yields a 3 to 4 times bigger femoropatellar joint compressive force for downhill walking compared to level walking. Due to a smaller moment arm of female subjects the patello femoral joint compressive forces were bigger for females compared to males (14 N/kg bw for male subjects, 18 N/kg for female subjects during level walking and 50 N/kg bw for male subjects, 70 N/kg for female subjects during downhill walking). The femoropatellar joint compressive force for downhill walking is comparable to running, squatting or downstairs walking. Based on this investigation downhill walking must be considered a strenuous task for the femoro patellar joint and explains well why patients with anterior knee pain or osteoarthritis have problems during downhill walking.
","PeriodicalId":77470,"journal":{"name":"Zeitschrift fur Unfallchirurgie und Versicherungsmedizin : offizielles Organ der Schweizerischen Gesellschaft fur Unfallmedizin und Berufskrankheiten = Revue de traumatologie et d'assicurologie : organe officiel de la Societe suisse de ...","volume":"86 3","pages":"178-83"},"PeriodicalIF":0.0,"publicationDate":"1993-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19122679","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
