{"title":"A new technique for functional surgery of the nasal valve area.","authors":"G Sulsenti, P Palma","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Nasal valvular obstruction is a neglected and disregarded clinical entity in current practice. Rhinomanometry and nasal endoscopy allow a better evaluation of the degree and the site of the stenosis. The distinction between nasal valve and nasal valve area is fundamental in surgical practice. Valvular disturbances are often induced or aggravated by rhinoplastic procedures due to scar tissue formation in the region of the nasal valve area and/or to excessive resection of supporting cartilaginous structures. Routine septal surgery is often followed by sequelae in the valve area structures. The treatment of valvular disturbances is basically surgical. A systematic surgical approach should comprise the structures of the nasal valve area as well as the structures functionally related to it. After a brief review of the techniques described in the literature, the authors present an original corrective technique for the valve area deformities, performed through the hemitransfixion incision. This technique results from previous experience founded on Cottle's surgical philosophy. This technique assures the correction of the valve area deformities as well as the performance of surgical manoeuvres extended to the osteocartilaginous pyramid and the nasal cavities. It is therefore possible to properly correct the areas of resistance and to re-establish adequate functional relationships between the various parts.</p>","PeriodicalId":77364,"journal":{"name":"Rhinology. Mean numbers of plasma- and mast cells were not different in the various groups. Immunoglobulin isotypes were always predominantly IgA and IgM; IgE were scarce. Deposited immune complexes were always absent. A statistically significant difference, however, was observed in the number of eosinophils within the mucosa. Patients with nasal polyps had up to ten times more eosinophils per surface unit than patients with sinusitis or healthy mucosa.</p>","PeriodicalId":77364,"journal":{"name":"Rhinology. {"title":"Recurrence of nasal polyps after surgical treatment.","authors":"S Jäntti-Alanko, E Holopainen, H Malmberg","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Recurrence of nasal polyposis after polypectomy or ethmoidectomy was studied in 85 patients four years after surgery. The patients were classified into one of three groups according to clinical findings: an atopy group (history confirmed by skin test or nasal provocation), an acetylsalicylic acid intolerance (ASA) group (confirmed by provocation), or an \"intrinsic\" group (no specific diagnosis). The risk of recurrence was significantly greater in patients with ASA intolerance than in the other two patient groups; the frequency of reoperations during the follow-up period was significantly higher in the ASA group and the need for topical corticosteroid treatment had also been more frequent. Bronchial asthma was diagnosed in 40% of all patients. Asthma was significantly more often associated with ASA group (91%) vs 46% at AT and in only 16% at INTR group.</p>","PeriodicalId":77364,"journal":{"name":"Rhinology.
