The differential diagnosis was considered in relation to other possible benign and malignant lesions where there is swelling of the mammary gland in men. US was the examination of first choice. To complete the study of the diagnostic pattern, mammography was also performed. Plain abdominal X-ray performed on emergency showed marked small bowel distention, and air-fluid levels suggestive of intestinal obstruction. CT was indicated to establish its precise site and cause. The presence of a gallstone was evidenced: gallstone ileus was diagnosed. Interestingly enough, at surgery the gallstone was not found; most likely it was expelled spontaneuously during the time elapsed between CT and surgery. No-contrast CT documented the presence of a neoformation approximately 3 cm in size in the 4th ventricle. For an in-depth diagnostic study of the lesion contrast enhanced MRI was performed. The examination detected a second minute (< 1cm) intraparenchymal nodule highly suspicious of metastasis from primary extra-cerebral neoplasm. The differential diagnosis of infratentorial lesions is discussed. Chest X-ray showed the presence of bilateral diffuse reticulonodular interstitial involvement. For better characterization HRCT was performed. It detected ground glass interstitial involvement associated with nodules predominant in posterior segments of bilateral lower lobes. The case history associated with the findings of diagnostic imaging are strongly suggestive for pulmonary infection. The patient came to the Senology Unit for annual control mammography. For diagnostic completion US was performed: it showed the presence of a hypoechoic neoformation highly suggestive of malignancy (class V BI-RADS). Therefore, US-guided needle aspiration biopsy was performed. It confirmed the malignant nature of the neoformation and surgery was planned.
