The purpose of this study was to explore the differences in perceptions and experiences in patients who have undergone both OPCAB and on-pump Coronary Artery Bypass Grafting procedures. Guided by naturalistic inquiry, semistructured qualitative interviews were conducted and audio taped with 4 men living independently. Inductive analysis of the transcripts revealed 7 themes: general comparisons, health care seeking symptoms, loss of control, faith and trust, postoperative depression, interactions with health care professionals, and sharing the life-time experience. The Australian society is rich in cultural diversity fostered by decades of migration. Both these factors present challenges for health care. First, because resources are scare in rural and remote regions, health outcomes are poorer in these regions, especially among indigenous populations. Second, the cultural diversity of Australians is a challenge to providing evidence-based treatment recommendations. {"title":"Correlates of fatigue in patients with heart failure.","authors":"Lorraine S Evangelista, Debra K Moser, Cheryl Westlake, Nancy Pike, Alvina Ter-Galstanyan, Kathleen Dracup","doi":"10.1111/j.1751-7117.2008.07275.x","DOIUrl":"https://doi.org/10.1111/j.1751-7117.2008.07275.x","url":null,"abstract":"<p><p>This study was conducted to determine the prevalence of fatigue and identify its demographic, clinical, and psychological correlates in 150 heart failure (HF) patients (73% men, 66% Caucasian, mean age 55.0 years, mean ejection fraction 26.7%+/-11%), from a single HF center, using the Profile of Mood States-Fatigue Subscale, the Minnesota Living With Heart Failure Questionnaire, and the Beck Depression Inventory. Sociodemographic and clinical data were obtained through self-report and chart abstraction. High levels of fatigue were reported in 50.4% of men and 51.2% of women. In a multivariate model, maximal workload, physical health, emotional health, and depression explained 51% of the variance in fatigue (P<.001). Fatigue in patients with HF is associated with both clinical and psychosocial variables, offering a number of targets for intervention. These findings suggest the need for multiple risk factor intervention strategies that improve physical and emotional health to decrease fatigue. This article examines the current state of research and practice for older adults and smoking cessation interventions. For over 17 years, the health care literature has mandated that people of all ages be provided with smoking cessation interventions. However, smoking cessation interventions are offered to older adults at suboptimal rates. Nurses, especially those caring for clients in the inpatient setting, have an opportunity to provide smoking cessation interventions to older smokers. Recent research has shown that older smokers, particularly those hospitalized with cardiovascular disease, can quit at high rates when provided an intervention. {"title":"Uncertainty as a predictor of adjustment to an implantable cardioverter defibrillator.","authors":"Ann Marie P Mauro","doi":"10.1111/j.1751-7117.2008.00008.x","DOIUrl":"https://doi.org/10.1111/j.1751-7117.2008.00008.x","url":null,"abstract":"Although the implantable cardioverter defibrillator (ICD) has reduced the risk of sudden cardiac death, some recipients experience uncertainty and adjustment problems related to the device. The study's purpose was to test hypotheses that predicted defibrillator recipients with greater uncertainty would have less psychosocial adjustment overall; less adjustment in the domestic, social, and vocational environments and in sexual relationships; and have more psychological distress at 1 and 8 weeks following hospital discharge. A repeated measures, correlational survey design was used with 94 adult defibrillator recipients from 4 hospitals. The Mishel Uncertainty in Illness Scale, Psychosocial Adjustment to Illness Scale Self-Report, and demographics were completed at both time points. The sample was predominantly white, male, and married. There were no gender differences and younger adults had more adjustment problems than older adults. Results supported all but 2 hypotheses as participants had less psychosocial adjustment overall, less adjustment in all domains except vocational environment, and more psychological distress at 1 and 8 weeks after discharge.
