{"title":"Study of access recirculation in reversed temporary indwelling catheters.","authors":"L Laurin,&nbsp;C Twolan","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>At the Ottawa Civic Hospital hemodialysis unit, our prescribed blood flow rate for temporary indwelling catheters runs at 200 mls/min. We were concerned that, with the reversal of the ports of these catheters, our patients were not receiving the dialysis prescription ordered for them due to decreased efficiency of their treatment. Twenty-five of 108 patients in the program were included in a study related to the percentage of access recirculation when ports were reversed or not reversed. Of those 25 patients, approximately 42% had their ports reversed. The study took place over a two-week period and involved approximately six dialysis sessions per patient. Results indicated that the percentage recirculation increased from 2.3%-4.1% when ports were connected normally, to 12% when ports were reversed. As a result, a policy is now in place whereby our patients' treatment time is increased 20 minutes for reversed ports. In summary, recirculation rate increased with reversal of ports in temporary indwelling catheters. In the ever-changing field of nephrology, most skills practised in nursing are considered to be sanctioned medical acts or added nursing skills. Therefore, annual recertification of the skills designated as sanctioned medical acts is an expectation of the College of Nurses of Ontario. The Wellesley Hospital policy indicates one time only or annual approval of the added nursing skills. The article will discuss the use of games as a creative, non-threatening educational tool in the recertification/re-approval process currently in place at The Wellesley Hospital, renal programs. In the past two years, several games or alternative teaching strategies have been utilized to assist the staff in preparing for recertification. This paper will examine the advantages and disadvantages of utilizing alternative teaching formats. {"title":"Reaching out: bringing the human factor back to dialysis.","authors":"J Olson,&nbsp;L Cielen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Prior to the early 1980s, two Winnipeg hospitals provided hemodialysis for all patients in Manitoba with chronic renal failure. Because no other hemodialysis centres existed, families were forced to relocate to the city. Because of these factors, the Manitoba Renal Failure Advisory Committee proposed the development of an outreach hemodialysis program. Under the auspices of the Health Sciences Centre in Winnipeg, this outreach program has evolved into the current Manitoba Local Centre Dialysis Program. Hemodialysis services are now available in an additional seven health care centres throughout Manitoba and northwestern Ontario. This program has benefited many and in some instances, families previously separated by distances of up to 500 miles have been reunited.
