Antibacterial effects of ofloxacin, clindamycin and sultamicillin on surgical removal of impacted third molars. 氧氟沙星、克林霉素和舒他西林对阻生第三磨牙手术拔除的抗菌效果。

{"title":"Antibacterial effects of ofloxacin, clindamycin and sultamicillin on surgical removal of impacted third molars.","authors":"K Göker,&nbsp;O Güvener","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>This study examined the bacteraemia following surgical removal of impacted mandibular third molars and evaluated the antibacterial effects of Ofloxacin, Clindamycin, Sultamicillin, used as prophylactic medications. The study involved a hundred healthy patients whose mandibular third molars were impacted horizontally. These patients were divided into four groups each including 25 individuals. One of the four groups was the control group. The other groups were those to which Ofloxacin, Clindamycin, and Sultamicillin were administered one hour before the operation and in the following 4 days postoperatively. Blood samples were taken before and immediately after the operation, and then, 1 and 24 hours postoperatively. Following the incubation of the samples under aerobic and anaerobic conditions, the samples were examined microbiologically. Preoperative samples were found to be negative. In the immediate postoperative samples, bacteraemia was found in 44% of the control group, 40% of Ofloxacin and Clindamycin groups and 36% of the Sultamicillin group. In the samples taken 1 hour after the operation, bacteraemia was found in 28% of the control group, 20% of the Ofloxacin group and 24% of the Clindamycin and Sultamicillin groups. In the control group, only 2 cases showed positive culture in the blood samples taken 24 hours after the operation. Twenty-four of them were early onset periodontitis (EOP) patients and 12 were adult periodontitis (AP) patients. Based upon the age, number of involved teeth and the location of diseased sites, the EOP patients were divided into two groups as juvenile periodontitis (JP) and rapidly progressive periodontitis (RPP). All the patients were individually examined both clinically and radiographically. At a baseline examination and 3 months after active therapy, the patients were evaluated for plaque, gingivitis, probing depths and probing attachment levels. Following instruction in proper oral hygiene measures, all of the patients were subjected to periodontal treatment. Treatment comprised scaling and root-planing, occlusal adjustment, tetracycline administration and periodontal surgery with osteoplasty. Results showed that age limits between JP and RPP cannot be drawn with certainty. The mean age for the JP group was 21.83 years and was 30.58 years for the RPP group. The number of involved teeth was found to increase with age. The first molars were the most frequently affected teeth followed by the incisors. The teeth were soaked in India ink and then decalcified, cleared and used in an in vitro study to determine the number of root canals and their types, the ramifications of root canals, the location of apical foraminae and the frequency of apical deltas. These parameters have been evaluated according to WHO criteria. Corresponding parameters have been statistically evaluated. When experimental and control groups were compared statistically, a highly significant difference was seen between df and DMF indexes of diabetic and healthy children. In the periodontal evaluation, a significant difference was found in the gingival index, but no such significance was seen in the periodontal index. From the results of this study it can be seen that children with juvenile diabetes mellitus had less caries incidence compared with the control group. A total of fifteen patients who had adult periodontitis and demonstrated bilaterally similar periodontal destruction participated in the study. After the initial examination, they received instruction in oral hygiene and had their teeth thoroughly scaled and root-planed. The posterior segments of these patients were then treated with osseous recontouring or flap curettage. Following treatment, the patients were recalled every 4 weeks for professional tooth cleaning. Post operative measurements were made at the sixth month. The results demonstrated that osseous recontouring and flap curettage equally reduced plaque and gingival inflammation. Although, osseous recontouring caused loss of attachment at the facial and lingual surfaces, probing attachment gain at the approximal surfaces in both treatment groups had occurred but this was limited (0.5 mm) and there was no difference statistically between the two groups. Pocket reduction was achieved in both treatment modalities and differences in the two groups were statistically significant. Since the most common topical fluoride application is via toothbrushing with fluoridated dentifrices, the aim of this study was to measure fluoride uptake by dental plaque after a single toothbrushing with monofluorophosphate (MFP) toothpaste and to follow the plaque until its fluoride concentration returned to the prebrushing levels. In a fluoridated area, plaque samples were collected from 26 children who brushed with MFP toothpaste, 22 children who brushed with a fluoride-free control toothpaste, before brushing and at intervals of 10 minutes, 1 hour and 2 hours after brushing. A combination fluoride electrode was used to determine the fluoride content of the samples. In conclusion, our study has shown that brushing with fluoridated water has a positive effect on the fluoride level of the plaque and the addition of MFP did not make a significant change, although it had a short term effect. Twelve freshly extracted, non-carious human molar teeth were used and approximately 25 mm2 flat occlusal dentine surfaces were prepared. Citric acid and LIV CENERA Liquid were applied for 10 and 30 seconds to the dentine surfaces. The results of our study indicated that: 1. Conditioning with LIV CENERA Liquid for 10 seconds was not effective in removing the smear layer on the dentinal tubules but application for 30 seconds was partially effective. 2. Although 10 seconds application of citric acid was not effective in removing the smear layer, 30 seconds application removed it completely, but harmful effects were observed on the dentinal tubules. 3. We have found that the incidence of periapical granulomas was 53.7%. The incidence of cystic formations within an apical granuloma was 16.7% and the incidence of radicular cysts was 25.9%. It was established that the incidence of radicular cysts rose as the extent of the lesion increased and that the incidence of radicular cysts was 100% when the lesion size was 200 mm2 or more. In the experimental group, instrumented root canals were obturated with isobutyl cyanoacrylate and gutta-percha by using the lateral condensation technique. In the control group instrumented root canals were obturated with gutta-percha only. The penetration of radioactive 131I was evaluated by an autoradiographic technique. No leakage was observed in the experimental group.
