Numerous techniques have been used to determine the need for contralateral repair. Two groups of patients underwent transinguinal diagnostic laparoscopy at the time of herniorrhaphy to assess the contralateral groin. Of 20 patients older than 24 months, only 1 (5%) was found to have a patent processus vaginalis. In a group of 21 patients ranging from 2 to 24 months, 9 (42.9%) were found to have a contralateral patent processus vaginalis. Twelve patients had a closed or slit-like peritoneum at the internal ring. Transinguinal diagnostic laparoscopy can be used to identify which children may benefit from contralateral exploration, as well as those in whom surgery can be avoided.
","PeriodicalId":77211,"journal":{"name":"Journal of laparoendoscopic surgery","volume":"6 Suppl 1 ","pages":"S15-20"},"PeriodicalIF":0.0,"publicationDate":"1996-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19800849","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"The use of laparoscopy in the management of the nonpalpable testis.","authors":"J W Brock, G W Holcomb, W M Morgan","doi":"","DOIUrl":"","url":null,"abstract":"

Fifty-eight children with a nonpalpable testis have been evaluated at our institution between 1988 and 1994. Laparoscopy was performed in these children to effect a treatment plan. The children range in age from 10 months to 14 years. Thirty patients had a nonpalpable left testis, 18 had a nonpalpable right testis, and 10 had bilateral nonpalpable testes. In 28 children, a testis was visualized. Two underwent laparoscopic orchiectomy and 26 underwent either a standard single stage procedure (11), a staged open procedure (3), or a laparoscopically assisted staged procedure (12). Attenuated vessels were seen in 26 patients and inguinal exploration revealed a viable testis in 6 patients and a testis remnant, which was removed, in 20 patients. Four patients had laparoscopic blind-ending vessels. Diagnostic laparoscopy affords the surgeon not only the ability to localize the testis with virtual certainty, but also the advantage of planning a highly successful treatment program. The position of the testis intra-abdominally and the length of the spermatic vessels can be easily evaluated to decide whether a staged approach versus a single stage procedure can be performed. The use of the KTP laser has made the two staged procedure quite easy with only one Trocar site necessary. This experience has allowed us to develop and modify an algorithm for the effective management of the nonpalpable testis.
","PeriodicalId":77211,"journal":{"name":"Journal of laparoendoscopic surgery","volume":"6 Suppl 1 ","pages":"S35-9"},"PeriodicalIF":0.0,"publicationDate":"1996-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19800852","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This technique allows a reduction in complications and sequelae. The majority of indications, such as renal dysplasia and destroyed kidneys due to obstructive or refluxing uropathy, are suitable for laparoscopic nephrectomy. Contraindications are Wilms' tumor and trauma, which represent only 20 percent of nephrectomies in our experience. As in open surgery, to perform nephroureterectomy for benign disease, a retroperitoneal approach seems more logical than transperitoneal approach, which is the usual approach for laparoscopic surgeons. We have attempted six retroperitoneal laparoscopic nephrectomies in children from 3 months to 14 years old. The patient is positioned in a lateral position after creation of a retropneumoperitoneum under visual control; three or four ports are needed and renal vessels are dissected then clipped, or coagulated if small. Destroyed kidneys are generally of small size, so they can be extracted via a 10- or 12-mm cannula site without morcellation. Operative time ranges from 35 to 210 mm (median 120 mm). We have had no complications or conversions. Retroperitoneal laparoscopic nephrectomy in children is a feasible and safe procedure in well-trained hands.
","PeriodicalId":77211,"journal":{"name":"Journal of laparoendoscopic surgery","volume":"6 Suppl 1 ","pages":"S55-9"},"PeriodicalIF":0.0,"publicationDate":"1996-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19800745","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

An experimental study.","authors":"S Z Rubin, G M Davis, Y Sehgal, M J Kaminski","doi":"","DOIUrl":"","url":null,"abstract":"

Notwithstanding the clinical acceptability of pediatric laparoscopy, the physiological consequences of carbon dioxide (CO(2)) peritoneal insufflation on lung mechanics and gas exchange are not established. This study seeks to establish the affects of increased intraabdominal pressure (P ABD) from CO(2) insufflation on lung mechanics, the compliance of the respiratory system (CRS stat), and gas exchange. Ten anesthetized tracheostomized, and paralyzed rabbits (2.5-3.1 kg) were examined. Mechanical ventilation was established to produce normal ventilation (PaCO(2) 35-45 Torr). Continuous pulse, blood pressure (BP), and serial arterial blood gas measurements were obtained by femoral arteriotomy. The animals were examined at baseline conditions (P ABD = 0 mm Hg), at 2 levels of external compression (P ABD = 6 and 12 mm Hg, respectively), and at two levels of intraperitoneal CO(2) insufflation (P ABD=6 and 12 mm Hg, respectively). Increased P ABD, whether applied externally or by intraperitoneal insufflation, did not alter the CRS stat (ANOVA p = 0.557), nor systematically change the pH, PaCO(2) or PaO(2) (ANOVA p = 0.541, p = 0.545, p = 0.446, respectively). Neither BP nor pulse rate showed any change. Under conditions encountered during laparoscopy, P ABD of 0-12 mm Hg did not have a deleterious effect on ventilation and gas exchange in the ventilated and paralyzed small animal.
","PeriodicalId":77211,"journal":{"name":"Journal of laparoendoscopic surgery","volume":"6 Suppl 1 ","pages":"S69-73"},"PeriodicalIF":0.0,"publicationDate":"1996-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19800748","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The indications were neurological disorders of swallowing in 15, head and neck cancer in 4, and head injuries in 7. No major complications were seen in this group. One episode of temporary peritubal leakage settled after catheter change. One tube blocked after 4 months of feeding and temporary loss to follow-up. At follow-up, the tube continued to function in 7 patients, a mean of 130 days (range 45-667 days). In 7 patients recovery was sufficient to allow catheter removal at a mean of 134 days (range 56-450 days). Twelve patients died of their disease, a mean of 76 days after insertion (range 43-300). Patient tolerance and ease of care have been excellent. There is a suggestion that recovery specially in head injury patients may be enhanced. A patient is reported here whose primary tumor appeared controlled by surgery and radiation, but who died of the disease after developing implant metastases at three untreated trocar sites. The second case report illustrates the difficulty in identifying gallbladder cancer during laparoscopic cholecystectomy, and the importance of a diligent preoperative effort to establish the diagnosis. Current literature suggests that tumor implantation occurring during laparoscopic cholecystectomy for inapparent carcinoma adversely affects prognosis, and, until the effect of laparoscopy on the spread of this tumor is better understood and controlled, open operation should be performed when carcinoma of the gallbladder is suspected. When laparoscopic cholecystectomy is done for inapparent gallbladder cancer, surgical and adjuvant radiotherapy to the trocar sites appears to improve outcome in association with extended treatment to the gallbladder bed and adjacent areas. Recent reports suggest that progress in diagnostic, surgical, and adjuvant techniques could substantially improve survival in carcinoma of the gallbladder.
","PeriodicalId":77211,"journal":{"name":"Journal of laparoendoscopic surgery","volume":"6 1","pages":"35-45"},"PeriodicalIF":0.0,"publicationDate":"1996-02-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1089/lps.1996.6.35","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19884857","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

One of the difficulties inherent to the procedure is the establishment of an adequate working space between the abdominal wall and the peritoneum. A quick and efficient method is reported here that was used in 15 consecutive patients. The method provides wide exposure without the use of any sophisticated equipment or the need for a significant learning curve. Exposure of the preperitoneal space with the dissection technique described simplifies a difficult first step and makes the extraperitoneal repair an easier undertaking.
","PeriodicalId":77211,"journal":{"name":"Journal of laparoendoscopic surgery","volume":"6 1","pages":"47-9"},"PeriodicalIF":0.0,"publicationDate":"1996-02-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1089/lps.1996.6.47","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19884858","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
