This pseudotumoral neoformation often results from a pathologic process of the pelvis (95%). Recurrent cystitis is the main symptom. The diagnosis is difficult and cystoscopy and biopsy is the basic investigation. Surgical treatment must remove the original lesion with partial or total cystectomy. Ultrasonic and CT scan examinations will suspect the diagnosis in the majority of cases. With the histological diagnosis and its prognosis, the treatment may be watchful waiting, partial nephrectomy or radical nephrectomy. Partial nephrectomy is indicated in case of multiple tumor in a patient with solitary kidney or for bilateral tumors. Urinary fistulas or renal insufficiency are potential complications, but they are exceptional. {"title":"[A case of lithiasis caused by 2,8 dihydroxyadenine. Evolutive characteristics. Therapeutic problems].","authors":"J Thomas, P Conort, D Fompeydie, J Bellin, J P Rechke, G Arvis","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>We report a case of bilateral 2,8 dihydroxyadenine urolithiasis in a 25-year-old woman, born from an incestuous union between a brother and sister, who developed the first manifestations at the age of 9 years. This condition results from APRTase deficiency and an autosomal recessive inherited disorder. Diagnosis requires physical analysis of the stones and measurement of APRT activity in red cells. Treatment by lithotripsy with piezo-electric shock waves was ineffective on the left leading to eventual nephrectomy. The right lithiasis was destroyed by lithotripsy. This result would suggest variable solidity of 2,8 dihydroxyadenine stones. In this patient, an indwelling endoprosthesis was left in place for 14 months for extra-medical reasons and led to staphylococcal urinary infections, struvite lithiasis and incrustations of the catheter within the bladder. He was noted to be icteric and his liver enzymes were elevated. Within a week of discontinuing the medication, the patient's mental status and liver function returned to normal. Although flutamide-induced near fatal liver dysfunction has been described, progression to encephalopathy is a rare occurrence. {"title":"[Electroconductive extracorporeal lithotripsy with Sonolith 4000+. A prospective study of the ECG desynchronization using a fixed-rhythm simulator].","authors":"T Flam, D Saighi, F Legagneux, Z Ounnoughene, D Duboc, N Thiounn, M Bourlion, M Zerbib, B Debré","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>Electrohydraulic lithotripters use a R-wave triggering system as episodes of cardiac arrhythmia were observed during early clinical experiments. This study was designed to assess the safety of non triggered shock wave treatments using an external fixed rate device (120/min) on the Sonolith 4000 Plus (*), a non bathtub electroconductive lithotripter.</p><p><strong>Methods: </strong>Our study is the first prospective evaluation of non-EKG-gated lithotripsy where the patient under continuous Holter monitoring served as his own control during a randomized alternation of periods with or without R-wave triggering. This design allowed for a direct comparison of the effect of EKG-gated and non-EKG-gated lithotripsy respectively. The sequence of the different periods was randomly assigned. The recordings were compared to a pre-treatment recording, the cardiologist ignoring the randomization.</p><p><strong>Results: </strong>25 consecutive patients without cardiac history have been treated for urinary stones. Ventricular and supraventricular excitability disorders have been noted in 7 patients, always during a non-EKG-triggered period. There was no correlation with stone location. No cardiac rhythm disturbances have been observed during triggered periods in the 25 patients. Auricular extrasystoles (0.1-0.5/min) were observed in 6 patients, and auricular couplets (0.5/min) in one. Ventricular extrasystoles were noted in 4 patients, and ventricular couplets in one. One patient had an asymptomatic non sustained ventricular tachycardia which resolved spontaneously. No clinically significant disorder occurred. There is usually no clinical expression of these cysts although development within the sinus can cause obstruction leading to acute nephretic colic. We observed two cases of intrasinus parapyelic cysts and another case of obstructive intrapyelic cyst. At ultrasonography, the only sign was hydronephrosis and a junction syndrome could not be eliminated. Intravenous pyelography demonstrated hydronephrosis and in one case a round intrapyelic mass. Computed tomography was required for diagnosis. Surgical exeresis was required due to the pain and compression damage to the parenchyma.
