{"title":"CPITN application in regular dental practice.","authors":"L Flores-de-Jacoby, R Mengel, U Joannou, G G Zafiropoulos","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The periodontal status of 8862 dentulous and partially dentulous inhabitants of the Federal Republic of Germany was investigated in various dental practices, using the CPITN, developed by the World Health Organization. The results show that a total of only 4.3% of those examined have a healthy periodontal apparatus and thus require no periodontal treatment. At least detailed oral hygiene instruction needs to be given to 95.7% of all subjects. Furthermore, 80% patients with periodontal disease require scaling or root planing, and 10% of them require periodontal surgery. The treatment of shallow gingival pocketing (CPITN 3) is predominant in every age group, and every fifth patient aged 35 years or more is need of periodontal surgery. This high frequency rate of periodontal disease illustrates the urgent necessity in regular dental practice for each and every patient to undergo periodontal screening. {"title":"[The pre- and intraoperative localization of superficially situated metallic objects in the head and neck area using a metal detector].","authors":"S Schultze-Mosgau, R Schmelzeisen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>For prevention of infections and with regard to forensic implications foreign bodies lost in the soft tissues should be taken out. A good alternative for the localization of metallic objects superficially situated in the soft tissues of the oral facial region, other than the use of radiographic means and stereotactic localization, is the use of a metal detector. This method can be used without complication before or during each operation demonstrated by two clinical cases. Its use avoids a large-scale tissue removal and exposure in the search of foreign bodies in superficial soft tissues. {"title":"[Mandibular condyle fractures in childhood--the clinico-roentgenological follow-up].","authors":"I S Altmann, K K Gundlach","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In a clinical and roentgenographical follow-up study in 35 children, who had suffered a fracture of the condylar process of the lower jaw, the following observations were made on the average 61 months after conservative treatment: The clinical outcome ranged from \"satisfactory function\" to \"restitutio ad integrum\". Best results were achieved after unilateral displaced fractures, while only few children showed a \"restitutio ad integrum\" after bilateral fractures or/and after fracture dislocations. In children, who were less than 8 years old, the results were much better than in older ones. Concerning the screening of oral carcinoma, the use of tumor markers is to be considered with criticism. The SCC-antigen seems to be the most useful for detection of recurrence in the follow-up. Two groups were formed with regard to the arrangement of the anterior teeth. After a mean period of wear amounting to 13 months different positions of the mandible were recorded in 21 patients under the guidance of a central bearing point and in occlusion. Patients with an arrangement of the anterior teeth free of interferences in excursive movements exhibited a significantly lower distance habitual occlusion/retruded position than subjects with an overlap limiting sagittal freedom. Above the tissue-specific energy threshold for photoablation the amplitude of the acoustic shock waves is proportional to the applied laser energy density. Laser energy densities of 2 J/cm2 cause pressure amplitudes of 50 bar, densities of 20 J/cm2 cause pressure amplitudes of 1000 bar. {"title":"[In-vitro research on the marginal adaptation of composites with the use of different dentin adhesives].","authors":"M Meissner, M Gatzer, D C van Gogswaardt, D Eichler, T Micheel, S Micheel","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The aim of this investigation was the check and evaluation of 7 various dentin adhesives regarding their influence on the connection between dentin resp. enamel and composite filling. For that cylindrical cavities in dental crowns (enamel cavities) and roots (dentin cavities) were prepared and treated with dentin adhesives and composite materials. One part of the test teeth was subjected to loads storage in aqueous solution and thermocycling. The marginal adaptation of the composite filling was checked by means of the dye penetration test. The evaluation was made after cutting the teeth in the stereo microscope TECHNIVAL 2 (Carl Zeiss Jena) at 16-resp. 25 magnifications and defined criteria. The fillings in cavities bound by enamel exhibited a very good marginal adaptation of the composite. The location of the dentin cavity (crown or root dentin) had no significant influence on the fitting of the filling. A clear penetration of dye could be observed with the fillings in the dentin with and without adhesives, which considerably intensified after thermocycling. Best results with regard to the marginal adaptation were obtained with Scotchbond 2, Mirage-Bond and Cavex-Clearfil-Newbond. {"title":"[The effect of the interocclusal space on temporomandibular recordings. The test persons performed mediotrusive and protrusive movements. These movements were recorded with tooth contact and with increased interocclusal space, with and without the clinician's supportive guidance. The objective of the study was to analyse the effects of increased interocclusal space on the recording of the condylar path. Statistical evaluation showed that neither procedure has any real advantage over the other. Both, the occlusal tray as well as the paraocclusal tray may, depending on clinical situation of the ligaments, lead to erroneous recordings. The reproducibility of the recording of the condylar path is only slightly decreased by increased interocclusal space.
