{"title":"[Postoperative analgesia after surgery of the knee].","authors":"D Fletcher","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>This review of postoperative analgesia after knee surgery provides a practical approach to the problem. The patient can be hospitalized or undergo day surgery, with different techniques being available for each situation. For day surgery, the intra-articular administration of local anaesthetic and/or opioids, the non-steroidal anti-inflammatory drugs can be used. During hospitalization patient controlled analgesia, epidural analgesia and peripheral nerve blocks as lumbar plexus, femoral or sciatic nerve block can be proposed. It must be emphasized that a combined approach using these different techniques may be useful in order to offer better analgesia and avoid some side effects. Each technique is described with advantages, side effects and practical usage its. The cost effectiveness of the transfusion therapy was not evaluated. To compare the different products, the cost of one gram of transfused haemoglobin was calculated. Acute normovolaemic haemodilution is the most economical technique. Intraoperative autologous transfusion without washing is the most expensive. {"title":"[Clinical, therapeutic and developmental aspects of acute bronchiolitis in Tunisia].","authors":"N Ben Jaballah, K Mnif, L Hedfi, A Hammami, F Khaldi, B Bennaceur","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Fourteen infants with severe acute bronchiolitis were admitted to the Intensive Care Unit (ICU) of Tunis. This pathology represents 36% of severe bronchopulmonary infections admitted to this ICU. Their age ranged between 2 and 48 weeks (mean: 15 weeks). Eight infants had hypotrophy. Two infants had congenital heart disease and one infant had tracheo-bronchomalacia. Viruses were found in 6/11 patients. Respiratory syncytial virus (RSV) was identified in five patients and an adenovirus in one patient. Five patients had respiratory arrest at ICU admission. Ten infants had evidence of atelectasis on chest X-ray films. Thirteen patients required mechanical ventilation. One infant had inappropriate antidiuretic hormone secretion resulting in convulsions. One infant had supraventricular tachycardia. Both had RSV infection. One patient who had congenital heart disease and RSV infection died. In the other 12 patients receiving mechanical ventilation, the mean duration of ventilation was 9 days (range: 2-30 days). {"title":"[Sequential anesthesia-analgesia].","authors":"J Brassier","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Combined spinal-epidural anaesthesia (CSEA) is realized by one route immediately before the surgical procedure. It consists of: -a spinal anaesthesia for the operation itself. -followed by the installation of a catheter in the epidural space, so as to ensure postoperative analgesia. CSEA is a simple and attractive technique which requires only a good knowledge of its two components by the anaesthetist and the use of appropriate needles. Main indications are low abdomen surgery and major procedures on the lower limb. Real contra-indications are few. {"title":"[Modified technique of Waters for retrograde intubation. Apropos of a case of difficult intubation].","authors":"G Runti, E Vicenti, F Casseler, G Clarich","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The authors describe a case of impossible intubation by classical means in an obese 37-yr old woman, affected with encephalopathy, hydrocephalus and severe psychomotor retardation. After failure of intubation by classical methods using a flexible introducer, the patient was intubated by Waters retrograde technique using an epidural catheter passed through the thyrocricoid membrane and recovered in the mouth. Following this guide the endotracheal tube passed the glottis fairly easily with the help of a flexible \"mouse tail-ended\" introducer (C-Mettro, Cook, 19.0.80) inserted into the same tube to facilitate alignment of distal tube and trachea.
