Sixty arthroscopic procedures in 58 patients were for sports-related injuries or symptoms. The most common diagnosis was maltracking of the patella. In 78 cases, an operative procedure was performed at the time of diagnostic arthroscopy. No complications were experienced. The accuracy of diagnosis for suspected meniscal tears was poor. Further pathologies, especially meniscal tears, were commonly associated with anterior cruciate ligament tears. One hundred and four intertrochanteric fractures of the femur were stabilized by external fixation over an 8 year period. Complete fracture healing was detected in all patients within 11.2 weeks. Varus malunions and more than 2 cm limb shortening were evaluated in 8 patients. Eight patients had permanent knee stiffness. A pin tract infection occurred in 13 patients. This technique is simple and safe. It allows the patients to mobilize earlier. The medial clavicle of each specimen was initially loaded in the anterior and posterior directions to provide control values for joint laxity. The anterior or posterior ligaments and capsular structures of the SCJs were then selectively cut and the specimens retested for laxity and then loaded to failure simulating either anterior or posterior dislocation. Testing of intact specimens showed that the posterior ligaments were stiffer than other structures in that it was significantly more difficult to posteriorly displace the SCJ than in any other direction and that the capsule was the important anterior structure affecting joint laxity. Load-to-failure testing showed that it required 50% more force to create a failure by posterior dislocation than by anterior dislocation. Six CP children (13.2 +/- 1.9 years) who showed moderate spasticity walked on a treadmill at relative speeds that ranged from slow to fast for three minutes at each speed. While walking with and without flexible plastic AFOs, oxygen uptake (VO2) was measured. Then an energy expenditure index (EEI) was calculated by taking the VO2 value divided by walking speed. A curve EEI-walking speed relationship was approximated to a parabolic curve. Then, the lowest value of EEI, as an economical EEI, was derived from the curve. In four of the six CP children, the EEI while walking with flexible plastic AFOs were larger than without flexible plastic AFOs. The economical EEI while walking with and without flexible plastic AFOs was 0.56 +/- 0.26 ml/kg/m and 0.42 +/- 0.14 ml/kg/m, respectively. The ambulation with flexible plastic AFOs required more energy than without flexible plastic AFOs (p < 0.05). We determined and analyzed by computer digitization the anterior and posterior acromial slopes for 141 cadaver shoulders (whose rotator cuff status had been previously assessed). No significant differences in either the anterior or posterior angle were found between the intact and rotator cuff tear groups. Frequency histograms of both angles demonstrated continuous, normal distributions. The individual acromions were consistently asymmetric in that the anterior slope was larger than the posterior slope. However, the anterior slope distribution did not reveal groupings indicative of the specific acromial types previously reported. Any element of the spine, the vertebrae, disks, surrounding soft tissues, potential spaces, or the cord itself may be involved solely, or more commonly, in combination. Staphylococcus aureus is the organism most frequently found, with Gram negative bacteria often cited in association with infections of the urinary tract and in intravenous drug abusers. Early diagnosis and treatment are essential for optimal outcomes. In some cases, the only necessary treatment consists of parenteral antibiotics and immobilization. However, certain instances require operative intervention. A case of spontaneous rib fracture in the third trimester presents the opportunity to examine factors unique to pregnancy that may predispose the patient to stress fractures of the lower ribs. A 28-year old woman in week 31 of her pregnancy presented with the chief complaint of acute onset of right upper quadrant pain. A chest radiograph demonstrated a minimally displaced fracture of the right 10th rib.
