Especially detection of primary tumor lesions is often difficult. Endoscopic ultrasonography is a relatively new imaging procedure localizing insulinomas preoperatively in about 90% of cases. Thus, previously used invasive preoperative imaging methods are usually unnecessary. The combination of endoscopic ultrasonography and somatostatin receptor scintigraphy allows visualization of gastrinomas in 90% of cases. Somatostatin receptor scintigraphy can also visualize metastatic lesions of gastrinomas and carcinoids in the whole body with high accuracy. In surgical management of a gastrinoma, duodenal transillumination and intraoperative ultrasound should be performed in all cases to exclude small duodenal or periduodenal, extrapancreatic tumors. US, CT, and MRI should be mainly used to exclude local and distant metastases. Angiography is helpful in detecting anatomical variations of abdominal vessels preoperatively. In 10-12% the right hepatitic artery arises from the superior mesenteric artery, the left hepatic artery in 4-5%. An aberrant origin of the common hepatic artery is seen in about 2.5%. We report about a patient with liver cell adenoma, in whom we diagnosed preoperatively the atypical origin of the common hepatic artery by performing CT angiography in the MIP technique (MIP = Maximum Intensity Projection). Nevertheless, the method has certain limitations, especially when employed to visualize extremely slow blood flow velocities, which occur e.g. in malignant tumors due to neovascularization. Recently, a new technique, the so-called angio-color procedure, has been presented, which is supposed to overcome the above-mentioned limitations. The way of coding the signal differs considerably from that in conventional color Doppler: The amplitude is color coded, not the frequency deviations. Results and first experiences with the new procedure in various domains were discussed at a 'Heidelberg Round Table' and are summarized in the present article. In the obstetrics/gynecology there are clear advantages in the representation of blood flow in the placenta. In eutrophic fetuses a blood flow over the entire placental breadth was detected, in dystrophic fetuses, however, only on the margins of the placenta. Conventional color Doppler could not visualize blood flow in the placenta at all. Blood flow assessment in the fetal organs also provided different results with the two procedures: the angio-color method showed color pixels even in the peripheral areas. In 8 malignant breast tumors the same observation was made, the angio-color procedure seemed to visualize more color pixels. In 4 of 6 benign lesions blood flow could be measured with the traditional color Doppler, but in 5 of 6 patients with the angio-color Doppler. This was also observed in ovarian tumors. Angio-color representation of renal transplants with normal function resulted in blood flow information reaching deep into the renal capsule, whereas in cases of renal transplants with impaired function a clearly reduced perfusion was visualized. These differences were not so evident with the conventional Doppler method. In lymph node diagnosis, too, marked differences between malignant and reactive lymph nodes could be visualized. In the diagnosis of arteriosclerotic plaques, blood flow in the residual lumen could also be detected more exactly with the angio-color technique. Color representation of liver tumors also showed a higher color signal density with ultrasound angiography. In summary, ultrasound angiography can provide additional information in the color representation of vessels with slow flow velocities. However, an obvious problem is the unability to quantify detected low blood flow. Continuous insonation and therefore determination of flow velocities in arteries are necessary to evaluate flow disturbances and changes of flow velocities during radiologic invasive interventions, i.e. implantation of stents, during percutaneous transluminal angioplasty therapy or local lysis. This modification of Doppler ultrasound examination is already used in neurosurgery for permanent monitoring of flow velocities in large brain vessels in one or both hemispheres during therapy. In our institute we developed a probe holder device to fix a miniature Doppler ultrasound probe (8-MHZ pulsed-wave probe) on the skin surface. Therefore continuous insonation during the manipulations is easy to realize. Information about complications such as occlusions, thrombi, emboli and changes in flow velocity is immediately present on the screen. We have monitored 40 patients during radiologic interventional therapy in arteries of the lower limb. An interactive of system findings leads to a more complete examination and evaluation, thus increasing the reliability and the objectivity of the examination. The standardized findings by use of text components mean at the same time a clear saving of time. Moreover, this system is very comfortable not only for the comparison of different images, but also for the scientific evaluation and processing of the image data. By digitalizing the sonographic images the visual data can be processed in a personal computer. Thereby, for maxillofacial surgery a simple and fast dynamic-functional evaluation of the temporomandibular joint was made possible. To review recent advances in diagnosis and treatment control of bleeding intestinal angiodysplasias with an endoscopic Doppler device.</p><p><strong>Design: </strong>Open prospective study involving 34 patients with bleeding from gastroduodenal and colorectal angiodysplasias.</p><p><strong>Interventions: </strong>In order to detect the superficial arterial vessels responsible for the bleeding, a total of 79 lesions were scanned by transendoscopic Doppler ultrasonography. 70 vascular ectasias (88.6%) were Doppler-positive and had injection therapy with epinephrine and polidocanol.</p><p><strong>Results: </strong>Out of the 70 sclerosed angiodysplasias, 63% (90.0%) could not be found endoscopically 2 weeks later, confirming the success of therapy. Doppler noise was still recorded in 7 visible malformations, indicating insufficient treatment. Further injections were made into these lesions, and the vascular anomalies were finally eliminated. During 1 year of follow-up, 2 of the 34 treated patients (5.88%) relapsed with actively bleeding cecal angiodysplasias. After repeated endoscopic hemostasis, no more hemorrhage was observed in both patients. The results were partly published in previous publications.</p><p><strong>Conclusion: </strong>Endoscopic Doppler ultrasonography may help in identification and treatment of intestinal angiodysplasias. However, practical experiences concerning reliability and efficiency of real PAC networks in radiological departments have to be made yet in order to improve network structure, software design and hardware performance. Important for surviving the trauma is a fast and efficient diagnostic procedure. The suspected diagnosis results from relatively typical criteria in the chest radiograph, especially the continuous widening of the mediastinum. The diagnostic procedure of 26 patients with polytrauma, abnormal chest radiograph and further examinations (intra-arterial digital subtraction angiography [i.a.DSA]: n = 10, contrast-enhanced CT: n = 19, CT and DSA: n = 5) was reviewed. Six patients had an aortic rupture. In 4 patients the diagnosis was confirmed by angiography, in 2 patients by surgery. The specificity was 100% for DSA and 40% for CT. To ensure the suspected diagnosis of a traumatic aortic rupture we therefore primarily recommend angiography.
