{"title":"Biomechanics of the Brånemark system.","authors":"B Rangert","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A dental implant restoration defines a biomechanical structure comprising both biological tissue, the bone, and mechanical components, the implant and the suprastructure. The relation between the applied load from the prosthesis and the supporting capacity of implant and bone will effect the long term outcome of treatment. Therefore, the biomechanical aspect is an essential element of the treatment planning. Related principles have been evolved through theoretical models, laboratory investigations, animal studies and in vivo measurements. Recently a number of clinical studies referring to implant treatment in partially edentulous jaws have been presented, revealing new aspects. It operates from a base of computer technology to enable the production, distribution and sharing of ideas. The use of multimedia in the educational setting is in keeping with the concept of learning, rather than teaching and, with this in mind, a pilot program in interactive computer learning has been developed which is geared specifically to the requirements of junior students. The design sequence and philosophy used in this program follow the teaching at Sydney University Dental School. Its purpose is to facilitate students in mastering the technical aspects of denture framework design, leading them through a set sequence of steps from saddle identification to finish. Although initially introduced for use in fully edentulous patients, their present widespread use in nearly all forms of tooth loss, as well as for the rehabilitation of maxillofacial prosthetics and in orthodontics, has significantly expanded the realm of restorative dentistry. The present multicentre retrospective study evaluates the results of 327 patients rehabilitated with 1047, 3i (Implant Innovations Inc.) threaded implants over a four-year period. Eight patients with 22 implants dropped out and were excluded from the study as they could not be evaluated. A total of 319 patients with 1025 implants remained. Nine implants failed in the mandible and 14 in the maxilla. There was a success rate of 98.2 percent in the mandible and 97.3 percent in the maxilla. The mean overall success rate was 97.75 percent, with little variation between the four study centres. This data further emphasises the trend in implant restoration that was becoming apparent at that time. However, the degree to which single tooth implant restorations now dominate clinical implant treatment is unprecedented. There is clear evidence of a progressive increase also in segmental and full arch bridgework, but minimal increase in overdenture restorations. The data also indicates that for the first time the number of males treated was greatest and that the maxilla was more frequently restored. This latter feature is no doubt closely linked with the emphasis given to single tooth restorations. The clinical features of various chronic muscle pain problems in the jaw region are discussed. Additionally, some of the objective findings of experimental studies which have utilized an acute experimental pain model (isometric contraction) in the jaw system are discussed. Some theories elucidated to explain chronic muscle pain are reviewed with emphasis on the possible role of the sympathetic nervous system which controls intramuscular blood flow. Blood flow studies are one way to evaluate post-contraction recovery of muscle to see if differences exist in subjects with or without chronic muscle pain. Methods to evaluate intramuscular blood flow during muscle contractions and some recent work looking at this issue are discussed. Although the primary issue in implant success is proper occlusal loading, prosthetic design and the distribution of the occlusal forces, one must also consider the importance of the maintenance of the implant environment. Understanding the implant environment and the rationale for impeccable bacterial control along with the principles for assessing the prosthesis and surrounding tissues may provide important information regarding the longterm success of the implant environment.
