{"title":"[The importance of psychosocial oncology in the treatment of cancer patients].","authors":"R Schwarz","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Better medical care leads to an increased life expectancy but also to a growing psychosocial burden on the patients and their relatives. This development has led to the establishment of a new discipline: \"Psychosocial Oncology\". The psychosocial support of patients and their relatives as well as of nurses and physicians is described using the psychosocial aftercare unit attached to the department of surgery of the Heidelberg University Clinic as an example. Generally speaking two types of co-operation are possible-the consultation model and the integrated liaison model. Of these the latter seems to be the most appropriate but also the one most vulnerable to dysfunctions caused by professional and personal rivalries. In this paper the conditions necessary for a patient centered co-operation are discussed. The subjects were distributed across the nine cells of a two-factorial design, factor 1 representing the degree of organic, factor 2 the degree of psychosocial risk. At age 3 months mother-child interaction was videotaped and evaluated with microanalytic technique. The quality of MCI could be demonstrated to be as predictive for later cognitive development as early developmental tests. Social-emotional problems of the toddler and preschool child was predicted better by parameters of early MCI than by temperament. Disturbed interactions were associated with later cognitive retardation and enhanced behavior problems. MCI is significantly determined by neglect and rejection, which showed typical behavioral patterns. The early separation of mother and high risk newborn itself did not lead to alterations in maternal attachment. {"title":"[Subjective illness theories: determinants or epiphenomena of coping with illness? A comparison of methods in patients with bronchial cancer].","authors":"H Faller","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The present paper analyzes subjective causal models in lung cancer patients. It addresses two questions: 1. What method is apt to assess subjective theories of illness? 2. What relationships can be found between subjective theories of illness on the one hand and emotional status and coping on the other hand. Subjective causes of the disease, assessed by a semi-structured interview and by a questionnaire respectively, are compared. Some connections with emotional status are tested. Univariate analyses show that certain causal models (\"air pollution\", \"psychological cause\") co-vary with emotional distress. This relationship, however, seems to be a result of a specific way of coping that works as a intervening variable, with which both subjective theory and emotional distress are confounded. The univariate connection can no longer be maintained when this way of coping is included in multivariate analysis. {"title":"[Prognostic indicators of psychogenic diseases in adulthood based on childhood variables].","authors":"G Reister, M Franz, A Wacker","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Using the data of the Mannheim Cohort Projekt (Schepank, 1987, 1990), the authors investigate possible correlations between psychic symptoms in childhood, risk and protective factors with regard to psychogenic disorders in adulthood. For this, the number of symptoms and the \"Impairment-Score\" as the degree of psychogenic complaints were related to each other by determining Spearmen's rank correlation coefficient. Additionally, the predictive power of these three variables were investigated by means of discriminance analysis. It could be shown, that the severity of psychogenic illness in adulthood increases with the number of childhood symptoms. There was no evidence for the hypothesis of an increasing predictive power for the assembled variables (number of symptoms, risk factors, and protective factors) in comparison with the single variables. Luborsky's Relationship Anecdotes Paradigm (RAP) Interview for which we introduced the German expression Relationship Episodes-Interview (RE-Interview) is a method for eliciting relationship episodes independently from therapy. In this paper we give an introduction into the practice and into the possible applications of this interview form according to the experiences the Ulm CCRT-group made with the instructions of Luborsky. The bases of our experience are 40 audio-visual recorded RE-Interviews.
