Color Doppler permits mapping of regional intrarenal blood flow, as well as flow in the main renal artery and vein. The spectral display of the Doppler signal arising from the renal arteries yields valuable information about the state of the peripheral vascular bed. Our understanding of intrarenal Doppler curves has evolved from a simplistic model of pulsatility index number equals diagnosis to the more sophisticated concept that changes in these curves imply a change in the physiology of the kidney. A retrograde urethrogram and voiding cystourethrogram can easily make the diagnosis unless the valve is located with the glandular urethra. Most are mucinous adenocarcinomas. The lesion's poor prognosis primarily reflects its clinically silent behavior in its early stages. Therefore, aids to preoperative diagnosis and tumor staging may be of benefit. Conventional radiographic techniques have not proved very effective at diagnosing urachal lesions. Magnetic resonance imaging (MRI) may be particularly useful in this regard, mainly due to its multiplanar imaging capability. TVS has improved the accuracy of evaluation of pelvic masses and affords a means for accurate guided aspiration. The accuracy of follicular monitoring and aspiration is also enhanced with transvaginal sonography. Monitoring of follicle growth and follicle aspiration is well accepted and commonly used. Defects of ovulation such as polycystic ovary disease or luteinized unruptured follicle syndrome may be recognized. Problems related to the union of the sperm and ova may also be seen. Obstructing lesions such as endometriosis and fibroids may be investigated by ultrasound. In males the principle use of ultrasound lies in the examination of the seminal vesicles, vas deferens, and ejaculatory ducts to see whether they are intact and unobstructed. {"title":"CT evaluation of active adrenal histoplasmosis.","authors":"E Levine","doi":"10.1007/BF02924600","DOIUrl":"https://doi.org/10.1007/BF02924600","url":null,"abstract":"<p><p>We describe three patients with puzzling systemic illnesses in whom computed tomography (CT) led to the diagnosis of active adrenal histoplasmosis. CT was performed in two patients because of gastrointestinal symptoms and weight loss and in the third because of fever developing 2 years after an apparent cure of histoplasmosis. All three patients had adrenal enlargement on CT with features of granulomatous adrenal disease in two. Hormone assays performed after CT supported the diagnosis of adrenal insufficiency and fine-needle adrenal aspiration biopsy with special stains of the aspirates showed histoplasma organisms in all three patients.
