{"title":"Perilymph fistula.","authors":"T O Wilcox,&nbsp;R A Hendrix","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Perilymph fistula, an abnormal communication between the inner ear and the middle ear, is an enigmatic otologic disorder which may present with auditory or vestibular symptoms. It is typically located at the oval or round windows, but several other locations have been suggested. It may be congenital or spontaneous, or due to trauma or surgery. It often poses a diagnostic dilemma because of the lack of a definitive test. Among the differential diagnoses of the perilymph fistula are Ménière's disease, infection, tumor and central nervous system disorders. Clinically, the definitive diagnosis of a perilymph fistula can only be made by exploratory tympanotomy. The etiology of pediatric cervical masses includes a variety of conditions. Unlike for the adult, there are few established guidelines for evaluation of these children. A recent review of 445 neck masses operated on at the Children's Hospital of Philadelphia noted the preoperative diagnosis to be correct in 270, or only 61%. The following points deserve emphasis: 1) the MHA-TP or FTA-Abs is recommended over the reagin serologic tests (VDRL, RPR); 2) one should regard this process as an attempt to define a subgroup of patients with sensorineural hearing loss who will benefit from a combination of antibiotics and steroid therapy rather than explicitly diagnosing infection of the temporal bone. To our knowledge, a threshold for movement detection in the affected hemifield has not been reported. A case of statokinetic dissociation from occipital lobe infarction is presented in which a threshold range for movement detection was identified. Along with its increased popularity has come concern over protection from the AIDS virus. Banked fascia lata is protected against the AIDS virus by donor selection, antibody testing and irradiation sterilization. With the knowledge that fascia may also be heated as additional protection, this study was performed to determine if heat treatment weakens the fascia.
