{"title":"Access to Dental Care Depends on Appreciation of Demographics and Economics.","authors":"H Barry Waldman, Steven P Perlman","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The evolving demographics and economics of dental practice require consideration as the profession plans forfuture changes in the dental workforce, the makeup of the population and ongoing access to care crisis. This paper is a sampling of U.S. Census Bureau reports on demographics and an appraisal of JADA and ADA News statements regarding the economics of dental practice, as well as a review of historical developments of the profession's perceptions of dental education and government support for dental care. The trends in economics and population-based changes affecting U.S. dental practice indicate a general downturn in practice busyness, stagnation of expenditures for dental services, and a continuing increase in the proportion in minorities throughout the country and the limited ability to make changes in dental economics without altering some long-held views on the delivery of dental services. Data were obtained from interviews with senior executives from several delivery organizations. Conducted by the senior author (HB), the summarized interviews were reviewed by informants. Oregon Medicaid enrollees now receive medical! dental care in capitated managed care organizations. Several dental group practices that provide care to privately and publicly insured patients are growing rapidly.The largest local dental insurer has diversified into other health products, including management services for affiliated dental practices. The Oregon dental market is undergoing a major reorganization: (a) large dental group practices are expanding and solo practices are declining; (b) all Medicaid patients receive their care in state-regulated Coordinated Care Organizations and their contracted Dental Care Organizations; and (c) more dental graduates are seeking employment in group practices. By defining ownership and equity interest, we are able to isolate a critical factor in the decision-making process. Evaluating the spectrum of the different models of dentistry gives us insight into potential advantages as well as disadvantages that can arise. In private practice, an owner typically wants to see the business grow over a lifetime, while, on the other end of the spectrum, corporate dental companies may be prioritizing dividends, investment returns through speedy packaged sales, or other means of reporting progress to investors. The dental component includes 24 dental clinics and a network of 2,500 dentists in a PPO plan, supported by a quality management team. An important feature of this network of clinics and dentists is the opportunity for pooling and analyzing data on oral health- care outcomes. These data are used to mentor the entire office team, to drive systemwide improvements in treatment protocols, and as part of providers' compensation. The question is what should a professional do when witnessing what appears to be a colleague's unethical behavior? A scenario is analyzed where a dental student observes what appears to be cheating on an examination.
