Eighty-two per cent of these patients improved and recovered, and required no further care. Eighteen per cent of these patients did not recover or experience significant improvement in their sensory or motor loss. This chronic group was treated by internal neurolysis. Ninety-five per cent of this chronic group treated by internal neurolysis improved. Only 5% did not have significant prolonged relief or improvement and eventually required excision of a sensory peripheral nerve. Internal neurolysis may not be indicated in those patients who suffer from transient sensory paresthesias; conservative care and/or an external peripheral nerve decompression may suffice. Internal neurolysis is indicated in those patients who have constant sensory aberations or motor loss from a chronic peripheral nerve compression syndrome or any other nerve lesion that has not responded well to conservative care or to an external nerve decompression procedure. Internal neurolysis, in addition to conservative care regimes presented here, has been found to be effective in reversing heretofore thought to be irreversible pain syndromes in the lower extremity even when significant atrophy has already occurred.</p>","PeriodicalId":76649,"journal":{"name":"The Journal of foot surgery","volume":"31 3","pages":"250-9"},"PeriodicalIF":0.0,"publicationDate":"1992-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"12785672","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Epineurial capping by microsuture has been shown to be somewhat effective in the reduction of the size of stump neuromas and in their confinement away from a weightbearing area. The present study shows that epineurial capping by coagulation of the epineurium with a radiofrequency wave Surgitron forms an effective seal around the proximal stumps of severed axons in rats. Although it has been demonstrated that the inter-evaluator reliability of the measurement of PASA is suboptimal, the measurement may still retain usefulness. If intra-evaluator reliability is acceptable and the measurement is a valid predictor of the intraoperative condition, then the measurement retains clinical usefulness. The present study was designed to determine the intra-evaluator reliability of the measurement of PASA. Five evaluators were asked to make the measurement on five radiographs. Measurements were repeated approximately every 2 weeks until a total of 12 measurements on each radiograph were complied. A Chi-square distribution test of variance was utilized to analyze the data. The results demonstrated that the intra-evaluator reliability of the measurement of PASA was acceptable in three of the five evaluators, suggesting that some physicians can achieve satisfactory consistency in the measurement. It is unclear why the intra-evaluator reliability of the measurement of PASA is suboptimal for some physicians. Further study is required to address this issue, as well as the issue of the validity of the measurement.</p>","PeriodicalId":76649,"journal":{"name":"The Journal of foot surgery","volume":"31 2","pages":"120-3"},"PeriodicalIF":0.0,"publicationDate":"1992-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"12811229","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
