Problems of self-care were reported as persisting in two thirds and restricted mobility in almost half of the patients. Outcome here was influenced by presence or absence rather than by side of hemiplegia. However, return to work was significantly more common with left hemiplegia. Memory defects were said to be displayed by one patient in three, communication loss and marked personality change by one in four. The assessment records whether the patient is dependent or independent in 20 items of activity. The items consist of six self-care tasks, six stages of mobility, three employment grades and five domestic activities. The assessment takes only a few minutes to record and can be completed at weekly intervals during staff conferences and routine clinical examinations. The trial included cases of various soft-tissue lesions and osteoarthrosis. Treatment was given to 170 patients, 84 of whom received fluproquazone and 86 the combination analgesic. The patients themselves made daily records of the severity of pain on a 100 mm visual analogue scale and in both treatment groups there was highly significant relief of pain, but with no significant differences between them. The haemoglobin concentration rose significantly within two months in 26 of the patients but this was followed by a significant fall to the pre-treatment level nine months after treatment. The response to iron therapy was not related to the initial haemoglobin concentration, serum iron concentration, transferrin saturation nor to the amount of storage iron, whether assessed by bone marrow stainable iron or the serum ferritin concentration. There was an unexpected fall in the serum ferritin concentration within the first two months after treatment in half of the patients and this was followed by a rise towards the pre-treatment level during the following seven months, such that there was no apparent addition to the amount of storage iron over the period of the study. The possible mechanisms for these findings are discussed. There were small reductions in erythrocyte sedimentation rate and IgG which did not achieve statistical significance. Rheumatoid factor titre did not change. The joint space was measured in radiographs of the knees of 120 patients with rheumatoid arthritis and correlated by computer analysis with age, sex, duration of disease, erosions, degenerative change, therapy and Quetelet's index (an index of obesity). Narrowing of both compartments of the knee correlated significantly with the duration of disease. Narrowing of the lateral, but not medial, compartment showed some degree of correlation with obesity as measured by Quetelet's index, but this did not achieve significance. The presence of erosions was associated with marked narrowing of joint space which frequently occurred early in the course of the disease; three-quarters of the patients showing even moderate erosions had at least one knee compartment narrowed to 2 mm or less and in half of these the changes had occurred in less than 10 years. There are various methods of attempting to measure pain in an individual. Pain threshold would appear to be the most useful of these as it is atraumatic and repeatable over long periods in healthy individuals. Various factors affect pain threshold such as anxiety. There are a number of ways of measuring pain threshold the requirement for these being similar, for example producing no tissue damage. Pain threshold appears to change in various clinical conditions such as osteoarthrosis of the hip once the painful condition is relieved the threshold may revert back to normal. Other painful conditions such as rheumatiod arthritis do not appear to alter the pain threshold.
