{"title":"[Immediate denture and bar retention denture: two solutions of choice to re-establish function and improve esthetics].","authors":"J M Rignon-Bret","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Possibilities given by our technics concerning immediate denture and contra-mucosal retention bar mandibular complete denture, allow, at present time, in five clinical steps, an esthetical improvement and a functional rehabilitation, realizing dental prosthesis with all the standard characteristics of classical removal dentures. Any transitional treatment may be suppressed, temporary prosthesis giving often rise to para-functions and resorptions. {"title":"[Focus on the smear layer in 1990].","authors":"L Scherman","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The smear layer is a debris-ladden layer, adhering firmly to the hard tissues of the tooth. It plays a major role in the different adhesive systems. An update of the recent knowledge on smear layer will be done in this article. When the carious lesion is removed, a microscopic debris-ladden layer forms at the level on the enamel and the dentine. This layer of materials is extremely adherent to the dental structure, and will come in between the hard dental tissues and the filling materials. In this article different studies were reviewed on the formation, structure and physiology of the smear layer. Consequences of this smear layer's removal on the pulp are discussed. During pregnancy, any radiography needs protection. Prescription must be strictly limited to the needs Xylocaine and Scandicaine 2% may be administered without danger, in small quantity. But the following lactation has to be stopped. Supine hypotensive syndrome which appears in the third trimester has to be known by the practitioner. {"title":"[Gerodontics and geriatric psychiatry].","authors":"J Richard","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Some of the important points in geriatric psychology and geriatric psychiatry (such as vocabulary and base line concepts, old myths in geriatrics, reference models, principle of action, therapeutic procedures, nasalgraphy, pluri, inter and transdisciplinarity) will be developed for the dentist practicing geriatric dentistry. Knowledge of these concepts should provide the basis for an effective association with the psychiatrist, in order to enhance better care for the elderly. Two types of approaches of the elderly, well known of the geriatric psychiatrist will be developed. This is why the impression of such a complex requires two distinct steps with two different modeling materials with different consistency. The first step is the border molding of the mucosal reflexion zone with a silicone of medium viscosity, placed on the borders of the custom tray. In this case report, adding to this confusing aspect, the patient's medical history and clinical signs led the investigations in a totally different direction.
