An analysis is made of the clinical and radiological findings, as well as the differential diagnosis of focal hepatic lesions. Due to the lack of clinical specificity in many patients, we recommend fine needle biopsy directed by ultrasound to confirm the suspicion of pyogenic hepatic abscess and isolation of noncausative microorganisms. In view of the few reports of this pathology in the Spanish and international bibliography, a new case that debuted as massive upper gastrointestinal hemorrhage is communicated. We reviewed the cases published in Spain to date, emphasizing the methodology of preoperative diagnosis, with special attention to the endoscopic signs characterizing gastric lipoma and computerized axial tomography, which is now viewed as the technique of choice. The diagnosis, which unfortunately is usually late, is based mainly on gastroscopy with biopsy and/or cytology. Treatment was initiated with a gluten-free diet and there was improvement. Later, in spite of the patient's compliance with dietary treatment, he suffered two episodes of intestinal obstruction. In the first episode the symptomatology remitted with intestinal rest, parenteral feeding and steroids, but clinical manifestations persisted in spite of medical treatment so exploratory laparotomy was realized. On pathological study were found ulcers that affected the mucosa and submucosa, inflammatory infiltrate without granulomas, and partial villous atrophy in the adjacent non-ulcerated mucosa, all compatible with a nongranulomatous ulcerative enteritis. {"title":"[Effectiveness of emergency endoscopic sclerosis in the control of digestive hemorrhage caused by gastroduodenal ulcer].","authors":"A Lanas, J L Alonso, I Alberdi, A Cornago, R Trujillo, M Lachen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>An investigation was made of the effect of emergency endoscopic sclerotherapy on the evaluation of digestive hemorrhage (HDA) secondary to gastroduodenal ulcer disease in two consecutive groups of patients. The control group included 92 patients and the sclerotherapy group contained 63. Both groups had the same management and basic treatment of hemodynamic stabilization, anti-H2 agents and alkaline . The sclerotherapy group also received a local injection of 1/10,000 (5-12 ml) adrenaline and 1% polydocanol (5-12 ml) if direct signs of hemorrhage (active bleeding, red clot, visible vessel) were seen at the time of early endoscopy. Surgery was indicated in the presence of persistent, recurrent or massive digestive hemorrhage. Thirty-two percent of the control group and 34% of the sclerosis group presented direct signs of hemorrhage at the time of endoscopy. Both groups were homogeneous with respect to sex distribution, NSAID intake, hemoglobin, presence of shock and etiology (33.3% and 36.3%, respectively, had duodenal ulcer). The average age was significantly higher in the control group than in the sclerotherapy group. Neither the presence of any endoscopic sign nor etiology contributed to the evolution of digestive bleeding. Seven groups were made according to the surgeons involved. Parietal, intraabdominal and general complications of each group were quantitated, as well as postoperative hospitalization and mortality.
