{"title":"[A method of stable osteosynthesis in diaphyseal fractures of the femur].","authors":"V V Kliuchevskiĭ,&nbsp;G Sukhanov,&nbsp;A S Bulatov","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The known methods of operative treatment of femur by means of intraosseous osteosynthesis, based on jamming of intraosseous structure in the bone, don't ensure sufficient level of the osteosynthesis stability that results in the secondary displacements. According to the method, proposed by the authors, a rod is introduced through the intercondylar fossa of the distal fractured fragment of femur (Imamaliyev, 1987) and is pierced through into the proximal fractured fragment of femur so that it perforates cortical layer of the bone in the region of greater trochanter with strong fixation in it. The structure is removed through the small incision in the area of the greater trochanter of femur. 35 patients have been operated according to this method. The remote results have been studied with 22 of them. Consolidation terms equal to 2.5-5 months. After antishock measures and primary surgical treatment with observation of its all elements, including application of delayed and tension sutures (31), the primary healing of wounds has been observed with 71, secondary--with 61, suppuration--with 23 and osteomyelitis--with 17 persons. 5 patients have been subjected to amputation of lower limb. 125 (77.5%) patients recovered and 33 (20.5%) became invalids. 3 patients have died. Successful treatment of these victims depends not only upon specialized but upon timely aid at pre-hospital stage as well. {"title":"[Biomechanical substantiation of osteosynthesis of certain fractures by the method of octahedral wire cerclage].","authors":"P V Nikitin","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In the process of experimental studies on 28 cadavers there has been stated the value of traction of the musculus quadriceps femoris, musculus triceps brachii, musculus gluteus medius, musculus peroneus brevis and calculated their arithmetical means. In the work there has been proven dependence between the muscle traction value and anthropometric data of the limb segment and the possibility of its calculation by means of the proposed formula.
