We examined bone metabolism by microdensitometry (MD) method in 94 patients with breast cancer (40 with bone metastasis and 54 without it) and 10 with benign breast diseases. There was no significant difference among the three groups in the background factors of menopausal status and hormone receptors status. MCI, GSmin, sigma GS/D and bone pattern by MD method in breast cancer patients with bone metastasis were significantly lower than those in the other groups. There were slight reverse correlation between sigma GS/D and serum ALP. MCI and sigma GS/D indices corrected by ages were also significantly lower in bone metastasis (p less than 0.01). These indices in most of the patients with bone metastasis gradually decreased as the lesion progressed. There were 14 cases of seminoma (S), 4 cases of E+Teratoma (T), 1 case of embryonal carcinoma (E), 1 case of E+STGC, 1 of S+E+T, 1 of S+E+ yolk sac tumor (Y) +choriocarcinoma (C) +STGC, 1 of mature teratoma, 6 of non-germ cell tumor. The stage of the germ cell tumors were stage I in 13 cases and stage II A in one case among 14 cases of seminoma, and stage I in 3 cases, stage II A in 3 cases, stage II B in one case, stage III in one case among 8 cases of non-seminoma. All of the patients underwent high orchiectomy. The seminomas were treated by radiotherapy and non-seminoma cases mainly by chemotherapy after retroperitoneal lymph nodes dissection (RPLND). Twenty one of 23 cases have survived, excluding one death (E+STGC, stage II A) and one unclear case. High orchiectomy and radiotherapy are the established procedures for the treatment of stage I seminoma. {"title":"Effects of etoposide (VP-16) on the survival and progression of cultured HeLa S3 cells through the cell cycle.","authors":"S Obata, Y Yamaguchi, T Miyamoto","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The survival of cultured HeLa S3 cells and their progression through the cell cycle after exposure to Etoposide (VP-16) were studied. The dose survival curve of exponentially growing cells was of a biphasic nature; the curve had an exponential slope starting from a small shoulder, and finally attained a plateau against increasing concentrations of the drug. Throughout the cell cycle, VP-16 exerted its main killing activity on cells in the late S and G2 phases, whereas cells in the G1 phase were resistant to the drug. School of Medicine from 1975 to 1989, with a review of 96 cases published in the Japanese literature between 1984 and 1988, and the following findings were obtained. 1. Eleven our 12 cases were acral lentiginous melanoma (ALM), and one case was nodular melanoma (NM). 2. The early two cases were died of distant metastasis. Of the recent ten cases which were underwent surgical excision combined with adjuvant immunochemotherapy with DTIC, ACNU, VCR and OK-432, nine cases had good prognosis, and one case was died of local recurrence. 3. {"title":"[Effective continuous intraarterial chemotherapy for a patient with FIGO stage IIIb cervical adenocarcinoma invasing the bladder wall].","authors":"Y Shimizu, H Teshima, I Fujimoto, K Hasumi, K Masubuchi","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A patient with FIGO stage IIIb adenocarcinoma of the uterine cervix (moderately differentiated, endocervical type) underwent an exploratory laparotomy because of a direct cancer invasion to the bladder wall, and then she was treated with consecutive intraarterial (IA) CDDP (10 mg/day) combined with continuous IA 5-FU (250 mg/day). Six weeks after, CR (complete response) was obtained by this IA chemotherapy (total dose; 5-FU: 10,500 mg, CDDP: 300 mg). Further 5-weeks-IA chemotherapy was added to keep the \"CR\" effect (final total dose; 5-FU: 19,250 mg, CDDP: 500 mg). The only toxic sign was a mild nausea. The patient's PS (performance status) was 0 (normal activity) and thereafter she could undergo a \"curative\" radical hysterectomy. The cancer invasion to the bladder wall observed at the first exploratory surgery completely disappeared histologically as well as macroscopically. In obtained material, small \"viable\" cancer focus was found in the cervical canal but the margin was free, and all nodes were negative. Postoperatively, the patient has receiving a continuous IA 5-FU (125 mg/day) for 13 months as a maintenance and she is free of disease (NED) with a normal activity (PS = 0). Patient characteristics were as follows. [table; see text] Age 43-77 (mean 63 years old) Endoscopic type Mean duration of PR was 26.6 weeks. The process of the improvement of primary lesion as judged by endoscopic findings were as follows. Firstly getting flat of wall, secondly reduction in size of ulcer, and lastly changing into scar. Number of reported cases including our case No. 6 which are diagnosed as scar endoscopically after chemotherapy and are operated successfully has been increasing. Most of them showed scar macroscopically with wide and irregular surface. Especially our case No. 6 showed keloidal scar. In these cases, the histological improvement into grade 2-3 was observed in scared tissue. Sooner or later, such a process of endoscopic improvement was observed 4-8 weeks after initiation of chemotherapy. {"title":"[In vivo anti-tumour efficacy of tumour necrosis factor and interferon- alpha, -gamma on human renal cell carcinoma heterotransplanted in nude mice].","authors":"T Onishi, T Machida, F Masuda, N Iizuka, K Nakauchi, N Furuta, H Shirakawa","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Using human renal cell carcinoma heterotransplanted in nude mice (JRC 11: anaplastic and alveolar pattern, grade IV), the in vivo anti-tumour efficacy of tumour necrosis factor (rHu-TNF-alpha: PT-050) and IFN-alpha (nHu-IFN-alpha: HLBI), IFN-gamma (nHu-IFN-gamma: OH-6000) were investigated. The dose of TNF was 1,000, 3,000, 10,000 J.R.U. (i.p., every day), that of IFN-alpha was 1 x 10(4) and 1 x 10(5) I. U. (s.c., every day) and that of IFN-gamma was 1 x 10(4) and 1 x 10(5) I.U. (s.c., every day). Mono-therapy of TNF, IFN-alpha and IFN-gamma was not effective with regard to tumour regression rates, histological degeneration rates and survival time of the host mice. Combination therapy of TNF and IFN-alpha, IFN-gamma were also not effective with regard to tumour regression rates, but when considering histological change, sporadic disappearance of endothelium of intra-tumoural vasculature, flow of tumour cells clustered in intra-vascular cavity, and extra-vascular bleeding were observed. With special reference to survival of host mice, prominent prolongation of survival time in combination treatment, especially in TNF combination therapy groups with the dosage of 10,000 J.R.U. was observed. Therefore we concluded that there is no synergistic reaction in combination therapy of TNF and IFN against JRC 11 tumour. {"title":"[Experimental and clinical study on injection of neocarzinostatin into the duodenal sub-serosa as chemotherapy for the regional lymphatic system in pancreatoduodenal cancer].","authors":"Y Hase","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Unlabelled: </strong>At the time of pancreatoduodenal cancer resection, Neocarzinostatin (NCS) was injected into the duodenal sub-serosa, as perioperative adjuvant chemotherapy for metastatic lymph nodes and cancer cells released into the lymphatic system. NCS content in the lymph nodes was measured after administration, and usefulness of the chemotherapy was investigated.</p><p><strong>Experiment: </strong>NCS (4,000 units) was administered into the duodenal sub-serosa of 18 adult mongrel dogs. NCS contents in the lymph nodes (those in the root of the mesentery and around the aorta) and pancreatic tissues were measured. NCS content in the pancreatic tissues decreased significantly 3 hours after administration, while that in the lymph nodes did not decreased even 3 hours later. Consequently, NCS is considered to be remain in the lymph nodes.</p><p><strong>Clinical findings: </strong>NCS (10,000 units) was administered into the duodenal sub-serosa in 18 resectable cases of pancreatoduodenal cancer. NCS content in the dissected lymph nodes was measured. The total number of dissected lymph nodes was 176, and the mean NCS content was 0.62 mu/g. The number of lymph nodes positive for metastasis was 14, and much content of NCS was observed in the specimens (mean NCS content: 1.25 mu/g). Examination according to site revealed the most content in the lymph nodes in pancreatic head closest to the site of administration, followed by the lymph nodes in the root of the mesentery and those in the hepatoduodenal ligament in the direction of lymphatic flow. Moreover, some distal lymph nodes also showed much content. Since progressive peritoneal dissemination was recognized, 150 mg of CDDP and 10 mg of MMC which were proved to be effective by chemosensitivity test of anticancer drugs were administered intraperitoneally. After one month, ascites increased. So LAK cells and TIL were transferred intraperitoneally 6 times. With this treatment ascitic collection remarkably decreased, the performance status improved and serum level of IAP and CA125 normalized.
