The percentage of frail older adults living in the community has increased as baby boomers age. The majority of these frail older adults are not edentulous, and require the services of a general dentist. Using a longitudinal case history, the authors have discussed the problems and decision making required to care for such an individual in a private dental office. The techniques for closure or obturation of any oro-antral/nasal communication are well described. What is less well described is the effective and timely restoration of facial form and dentition, ideally prior to commencement of radiotherapy. This paper describes the use of the zygomatic implant perforated (ZIP) flap technique to provide immediate reconstruction and rapid dental rehabilitation in three cases of midface/maxillary oncologic ablative defects. Three patients who underwent ZIP flap reconstruction at St. James’s Hospital between September 2022 and March 2023 are described. One of them had a total maxillectomy and two had an extended hemi-maxillectomy. All patients had a ZIP flap including a radial forearm free flap, with four zygomatic implants in one case and two zygomatic implants in each of the other cases. All implants placed had good primary stability and the median time to prosthesis fit was 22 days. To evaluate the effect of prioritisation and earlier treatment of these patients on treatment duration and patient compliance. The prioritised treatment group (PTG) had twin block appliances fitted when they were 11 or 12 years old. In comparison, the routine treatment group (RTG) had twin block appliances fitted when they were 13 or 14. Electronic patient records were screened to find a sample of 100 patients from each cohort. The number of compliance issues, functional appliance treatment duration, fixed appliance treatment duration and total treatment duration was recorded retrospectively from patient charts. While mean total treatment time was similar across the two cohorts, the PTG did have significantly shorter fixed appliance treatment durations than the RTG. The PTG was also found to have better compliance than the RTG. Prioritising patients who require functional appliance treatment does not reduce total treatment duration. A correlation between earlier treatment, improved patient compliance, and a shorter course of fixed appliance therapy can be seen, which may prove clinically beneficial. While considered normal in early childhood, the persistence of these habits has the potential to cause adverse effects on dental and skeletal relationships in the permanent dentition. Several interceptive techniques to promote habit cessation have been recommended. To update the dental practitioner on techniques recommended for cessation of NNSH in children and evidence of their efficacy. There are various interventions for NNSH cessation, including pacifier substitution, behavioural modification, orthodontic appliances, or aversive therapy. Psychological interventions were more likely to stop NNSH versus no treatment in the short and long term. Appliances were shown to be effective in stopping NNSH and demonstrated favourable effects on occlusion. There was no difference between alternative behavioural techniques. While there is insufficient evidence in support of a single intervention, these techniques demonstrate success when used alone or in combination to promote the cessation of NNSH. Creating awareness of the treatment options available for managing NNSH may allow practitioners to work with families to establish early habit intervention, with options individualised to the patient. Although highly conservative of tooth structure, the literature would describe composites as a ‘medium-term’ treatment option, with high patient satisfaction. Their survival has been quoted at five years when used in the treatment of tooth wear at an increased OVD.4 The evidence relates to paste composites, which were not nano-filled. It is clear that there is plenty of scope for research on nano-filled injectable composites in tooth wear cases. Radiotherapy (RT) as a primary or adjuvant management strategy can result in osteoradionecrosis (ORN), salivary gland hypofunction, and can contribute to dental caries, among other sequelae. Assessment by a dentist prior to RT is important to remove suspect teeth in the field of radiation and commence preventive and restorative care for the long term. This can reduce subsequent ORN risk, reduce oral complaints during RT, and optimise oral health-related quality of life (OHRQoL). This is a large-scale cohort study investigating dental treatment provided to a group undergoing RT for primary tumours of the head and neck. A retrospective chart review was carried out of all patients who had presented at the pre-RT dental clinic at Dublin Dental University Hospital from 2018 to 2019. A total of 490 patients were included. The mean age at presentation was 63 years (SD = 11). Approximately 50% of patients required removal of at least one unit and 12.7% of patients required basic dental restorations prior to commencement of RT. Most patients (93%) were assessed and treated within the requested timeframe. Pre-RT assessment of HNC patients is important, as many require dental treatments such as extractions or basic restorations. Barry","doi":"10.58541/001c.82024","DOIUrl":"https://doi.org/10.58541/001c.82024","url":null,"abstract":"Temporomandibular disorder (TMD) represents a common group of disorders related to the impairment of the temporomandibular joints and the associated neuro-muscular system, which commonly present with features such as pain in the orofacial region, headache, joint sounds, and disturbances in jaw movements. Recently, botulinum toxin type-A (BTX-A) has increasingly been used as an adjuvant treatment for TMD. This retrospective study aims to evaluate the effectiveness of BTX-A for the management of myogenous TMD and its potential to be included as a first line of treatment. A retrospective search was carried out through the Oral and Maxillofacial Department’s logbook from January 1, 2016, to December 31, 2020. Patients who received BTX-A for the management of myogenous TMD were identified and their hospital electronic records were accessed. From January 1, 2016, to December 31, 2020, 60 patients were diagnosed with myogenous TMD and treated conservatively together with intramuscular injections of BTX-A. Forty-five patients (75%) reported improvement in pain levels, of whom 10 (17%) reported complete resolution of pain. Fifteen patients (25%) reported no improvement in pain levels, of whom four (7%) reported transient improvement in pain levels lasting four weeks. A mean improvement of 50% was reported in terms of self-perceived pain levels. No adverse effects from BTX-A treatment were reported. Along with this we have seen a reduction in tooth loss. Therefore we will be managing increasing numbers of partially and fully dentate older adults in dental practice. Older adults in Ireland have high levels of past caries experience and, in particular, root caries is almost exclusively a disease seen in this cohort.
