An abdominal computed tomographic (CT) scan was obtained to look for a possible source of infection prior to repeat surgery. The CT scan revealed bilateral homogeneously enlarged hyperdense adrenal glands interpreted as representing adrenal hemorrhage. A diagnosis of acute adrenal insufficiency was suggested by the radiologist and appropriate therapy instituted. The adrenal hemorrhage was felt to be secondary to surgical stress in a patient with marginal adrenal function because of prior glucocorticoid use. The clinical features, etiologies and CT findings of adrenal hemorrhage and insufficiency in an adult are described.</p>","PeriodicalId":76034,"journal":{"name":"Journal of the Canadian Association of Radiologists","volume":"36 1","pages":"58-60"},"PeriodicalIF":0.0,"publicationDate":"1985-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"15092795","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The diagnosis was proven in four by splenectomy or autopsy, in three by comparison with CT, having been obtained shortly before infarction, and in two by angiography. In four patients the infarct occurred as a result of hepatic or splenic artery embolization. The CT features of infarction may be difficult to differentiate from those due to trauma, inflammation or tumor, and include either a diffuse, a well circumscribed or an irregularly marginated area of low density within the spleen. Classical wedge-shaped defects are occasionally seen in the periphery of spleen. The ultrasonographic feature is usually that of a hypoechoic or an anechoic area in the spleen. In one patient, it became necessary to perform an ultrasound guided aspiration biopsy to rule out splenic abscess.</p>","PeriodicalId":76034,"journal":{"name":"Journal of the Canadian Association of Radiologists","volume":"36 1","pages":"29-33"},"PeriodicalIF":0.0,"publicationDate":"1985-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"15001509","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Using a 27 cm image intensifier, these two important areas for atherosclerotic involvement were seen together on 68.1% of the images. In 25.6% of the images, the centering was too low to include the siphons on the film due to incorrect interpretation of facial anatomy on the oblique scout view. Careful attention to anatomic detail will allow correct centering in most patients. This will allow the bifurcations and siphons to be seen in profile together, thereby decreasing the number of film runs needed and the contrast volume given.</p>","PeriodicalId":76034,"journal":{"name":"Journal of the Canadian Association of Radiologists","volume":"36 1","pages":"37-40"},"PeriodicalIF":0.0,"publicationDate":"1985-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"15001511","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Image quality measurements of cardiac volumes, and ejection fraction and evaluation of regional wall motion were compared following digital subtraction and cine ventriculography in 30 patients undergoing coronary arteriography for suspected or proven ischemic heart disease. At concentrations of 13% to 19% (iodine contents: 2.0 to 5.0 g) meglumine sodium diatrizoate used for digital subtraction ventriculography, no alterations of ventricular pressures were noted; premature ventricular contraction and cineventriculography in 30 patients undergoing coronary arteriography for suspected wall motion by two independent observers showed complete agreement between digital and cineventriculography in 141 of 150 segments (94%), including 114 normal segments; digital ventriculography led to underestimation of four segments and to overestimation of five. Left ventricular ejection fraction and volumes calculated using both techniques correlated relatively well (ejection fraction: r = 0.86; end systolic volume: r = 0.88; end diastolic volume: r = 0.665). Thus, intraventricular injection of low concentrations of contrast medium provides good digital angiographic images and may eventually replace cine ventriculography for invasive assessment of left ventricular performance. However, ECG gating and shorter exposure times will be required for accurate measurement of ventricular volumes and ejection fractions.</p>","PeriodicalId":76034,"journal":{"name":"Journal of the Canadian Association of Radiologists","volume":"35 4","pages":"343-8"},"PeriodicalIF":0.0,"publicationDate":"1984-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"17456842","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Ioxaglate did not cause any major adverse effects; side effects were rare and mild. Hemodynamic changes were minimal, even in patients with severe coronary artery disease, and no arrhythmias were seen. The radiographic quality of angiograms was scored as good to excellent for each examination, and was equal to that of films performed using a conventional contrast medium.
