Such disadvantages can be minimized by joining the bony segments with tandem screws with threads in both parts of the bone. This technique avoids compression at the cut surfaces and permits contact and distance fixing by screws which are adapted to the course of the split. Since 1981, 132 mandibular osteotomies have been performed; they have proved to be adequately stable. The operative technique, the instruments used and the results of the follow-up are described. In order to evaluate the results, an index, namely the joint, relapse and sensation index is proposed for classification. This index can readily be used in practice.</p></div>","PeriodicalId":76010,"journal":{"name":"Journal of maxillofacial surgery","volume":"14 ","pages":"Pages 311-316"},"PeriodicalIF":0.0,"publicationDate":"1986-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/S0301-0503(86)80314-9","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"14590335","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is concluded that there are two types of condylar hyperplasia. The first manifests itself in the adolescent or the young adult and represents an exaggerated, normally proceeding growth and maturation process. The histological structure of the condyle in these cases is age-dependent as is shown by a conversion of hyaline growth cartilage into fibrocartilage occurring at about 20 years of age. The second type of condylar hyperplasia, seen in older people, probably represents reactive growth as a response to an eliciting agent that mostly can be identified. In these cases the histological architecture of the condyle is distorted by large masses of hyaline cartilage while there are concomitant degenerative changes in the form of arthrosis. In contrast to the patients suffering from the first type of condylar hyperplasia, the latter ones complain of additional joint symptoms, mostly pain. Moreover it is concluded that joint scintigraphy does not clearly discriminate between genuine condylar hyperplasia and reactive processes due to arthrosis, which may have clinical implications.</p></div>","PeriodicalId":76010,"journal":{"name":"Journal of maxillofacial surgery","volume":"14 ","pages":"Pages 209-214"},"PeriodicalIF":0.0,"publicationDate":"1986-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/S0301-0503(86)80291-0","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"14585568","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose of this study was to undertake a retrospective analysis of 40 cases of low-velocity handgun injuries of the maxillo-facial region. The majority of cases were due to assault followed by accidental shootings and failed suicide attempts. Injuries ranged from mild soft tissue damage without fractures to severe, comminuted fractures of the facial bones. In some cases, soft tissue damage was severe. Rarely were injuries life threatening. Entrance wounds were characteristically small and well circumscribed. In 60 per cent of cases the bullet did not exit and was retained within the tissues. The floor of the mouth and tongue were the soft tissues most commonly injured. Other structures injured were major blood vessels, nerves and the eye. Mandibular fractures occurred in 58 per cent of cases, followed in frequency by maxillary complex fractures. Twenty per cent of fractures simultaneously involved bones of the upper, middle and lower-thirds of the facial skeleton.</p><p>The above injuries are discussed and a classification of low-velocity hand-gun injuries of the maxillo-facial region is proposed.</p></div>","PeriodicalId":76010,"journal":{"name":"Journal of maxillofacial surgery","volume":"14 ","pages":"Pages 26-33"},"PeriodicalIF":0.0,"publicationDate":"1986-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/S0301-0503(86)80254-5","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"14580663","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A circular saw with an extremely fine saw blade is used. The ensuing cut is thus extremely narrow and the excised bone lid can be reimplanted exactly, flush with the neighbouring bone from which it was excised. Primary healing takes place without the development of intermediary connective or cartilagenous tissue. Only circumscribed pathological lesions of the diseased mucous membrane are removed. Otherwise, healing is promoted by improved drainage and ventilation. For this purpose a channel leading to the nose is created. In order to ensure that this channel stays open it is lined by dura or mucous membrane. The latter is put into place and fixed by an inflatable balloon acting as a tampon. The balloon is traversed by a tube which guarantees adequate drainage and ventilation during healing of the lining material in the channel.</p></div>","PeriodicalId":76010,"journal":{"name":"Journal of maxillofacial surgery","volume":"14 ","pages":"Pages 34-39"},"PeriodicalIF":0.0,"publicationDate":"1986-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://sci-hub-pdf.com/10.1016/S0301-0503(86)80255-7","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"14580664","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
