{"title":"Psychotic pica, nicotinism, and complicated myocardial infarction.","authors":"J F Neil, T L Horn, J M Himmelhoch","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The authors report a case of a schizophrenic patient who repeatedly consumed a wide variety of inedible materials, including significant quantities of tobacco. The phenomenology of this behavior, as well as its psychiatric and medical complications are discussed. It is probable that chronic nicotinism contributed to this patient's refractoriness to psychiatric treatment and to his eventual cardiovascular crisis. The occurrence of pica as a manifestation of severe psychopathology was vividly described by the early pioneers of neuropsychiatry, but has received little attention in recent psychiatric literature. This is in marked contrast to the syndrome of psychogenic water intoxication which continues to be reported frequently. The majority of descriptions of pica have dealt with its occurrence in children, in pregnant women, and as a societal practice in certain cultures studied from a medico-anthropologic point of view. The toxic organic brain syndrome caused by chronic ingestion of nicotine-containing products has also been neglected in psychiatric publications. Descriptions of the neuropsychiatric complications of subacute and chronic nicotinism have been restricted to textbooks of toxicology, where greater emphasis has been given to the acute effects of large quantities of nicotine, often in forms other than tobacco. Involvement of Spinal Cord is less constant. Drug intoxication should be excluded. An electroencephalogram may be done but what is paramount is the physician's clinical judgment. The medico-legal implications of brain death is less complicated than it is generally presumed to be. It is extablishment of the legality of brain death concept that is crucial. {"title":"The effects of long-term psychotherapy on patients' self-perception.","authors":"S Vora, W A Layman, E T Mann, A Danesino","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A review of the literature regarding the effects of long-term psychotherapy indicates that it has rarely been examined purely from the standpoint of the patient. This study attempts to determine how the patients perceive attitudinal change in themselves and their therapists during the treatment process. Twenty patients who received dynamically oriented psychotherapy in excess of one year in a typical outpatient clinic participated. These patients were randomly assigned a therapist who was either a Clinic staff member or senior resident in psychiatry. Slightly more than half the patients included in the study had a diagnosis of psychoneurosis, while the remainder were viewed as possessing characterological or psychotic disturbances. Evaluation of patient attitudes was achieved through the use of a modified version of Osgood's Semantic Differential Scale. It was determined that patients perceived themselves as becoming significantly more like their therapists after one year of psychotherapy. They saw themselves as more active, independent, dominant, outgoing, realistic and happy. Results in these patients afford further evidence that combined use of haloperidol and lithium is safe and effective in the treatment of mania.
