Merrot","doi":"10.1016/j.aorl.2008.11.001","DOIUrl":"10.1016/j.aorl.2008.11.001","url":null,"abstract":"<div><h3>Objective</h3><p>Describe the management of laryngeal pseudotumor in a young boy with exclusive endoscopic resection with laser CO<sub>2</sub> resection.</p></div><div><h3>Material and method</h3><p>Case report.</p></div><div><h3>Results</h3><p>We report a rare case of a laryngeal pseudotumor in a child that was successfully treated with endoscopic resection with laser CO<sub>2</sub> vaporization. The last follow-up at 12 months revealed no evidence of recurrence and no voice sequelae. This is the first case reported that was successfully treated after a single procedure. Merrot","doi":"10.1016/j.aorl.2008.12.002","DOIUrl":"10.1016/j.aorl.2008.12.002","url":null,"abstract":"<div><h3>Objective</h3><p>Describe the management of bilateral hypopharyngocele in a trumpet player.</p></div><div><h3>Material and method</h3><p>Case report of a professional trumpet player suffering from bilateral hypopharyngocele.</p></div><div><h3>Results</h3><p>A professional trumpet player was referred for an ORL consultation after he noticed that a bad sound was produced while he was playing associated with cervical pain. A CT scan demonstrated an asymmetric bilateral hypopharyngocele with no other abnormality.</p><p>Because of the mild symptoms and the professional context, a successful conservative approach was used with antireflux medications. He was advised to seek professional instruction in order to improve his technique. On follow-up examination 6<!--> <!-->months later, he had had no further problems since adopting the corrected techniques.</p></div><div><h3>Conclusion</h3><p>Pharyngoceles are rare and easily misdiagnosed. However, they continue to show a high incidence in developing countries.</p></div><div><h3>Objectives</h3><p>The aim of this study was to determine the impact of these complications in Morocco and to study the epidemiological, clinical, paraclinical and therapeutic patterns.</p></div><div><h3>Patients and methods</h3><p>This is a retrospective study conducted between January<!--> <!-->1998 and December<!--> <!-->2005. All patients presenting to the ENT department of the hôpital du 20 Août of Casablanca, Morocco, with complications of chronic otitis media and hospitalized during this period were included.</p></div><div><h3>Results</h3><p>We selected 47 out of 824 patients hospitalized for chronic otitis media, for a 5.7% incidence of complications, including 15 intracranial complications (1.8%) dominated by meningitis (nine cases) and cerebral abscess (five cases), and 32 extracranial complications (3.8%) dominated by mastoiditis (21 cases) and facial paralysis (eight cases). The cholesteatoma was found in 70% of the patients. The open atticomastoidectomy was the surgical technique chosen in 80% of the cases, associated with specific treatment depending on the complication. She developed respiratory distress and required endotracheal intubation.</p></div><div><h3>Results</h3><p>The endoscopy demonstrated a great deal of crust and pseudomembrane detachment. Bacterial culture grew group A β-hemolytic streptococcus. Progression was good with antibiotics and corticosteroid treatment.</p></div><div><h3>Conclusion</h3><p>Bacterial acute pseudomembranous croup (laryngotracheobronchitis) is rare. We must search for the most frequent diagnoses such as diphtheria and epiglottitis. Righini","doi":"10.1016/j.aorl.2008.10.001","DOIUrl":"https://doi.org/10.1016/j.aorl.2008.10.001","url":null,"abstract":"<div><h3>Objective</h3><p>The purpose of this study was to present an advanced case of rhinophyma and to discuss the pathophysiology, clinical presentation and treatment of this pathology.</p></div><div><h3>Case report</h3><p>A 75-year-old patient presented to the ENT out-patient department with a large rhinophyma causing nasal obstruction and eating problems in addition to its aesthetic social burden.</p></div><div><h3>Results</h3><p>The patient was operated under antibiotic coverage. The operated site was left to heal and later covered with a full-thickness skin graft. During the 5-year postoperative follow-up period, there were no complications, no recurrence of the rhinophyma, and the patient retained a good aesthetic result.</p></div><div><h3>Conclusion</h3><p>Rhinophyma is the highest of the four grades of rosacea progression. The diagnosis is usually clinical. When this is demonstrated, each surgical variant can then be undertaken with the surgical landmarks described below.</p></div><div><h3>Results</h3><p>The classical suprapetrosal approach allows one to directly control the entire internal acoustic meatus from the fundus of the canal to the <em>porus</em>. Enlarged, it provides access to the cerebello pontine angle nearby to the <em>porus</em>. Finally, the petrous apex can be approached while preserving the bony labyrinth via the transpetrosal middle fossa approach.</p></div><div><h3>Conclusion</h3><p>Difficult to learn, the middle fossa approach depends on recognizing the surgical landmarks of the anterosuperior surface of the petrous bone.
